摘　　要：目的探讨快速康复外科(FTS)理念在老年髋关节置换术围术期的应用效果。方法选取2018年1月至2018年12月在我院行髋关节置换术的老年患者117例为研究对象,将其按入院时间的先后分为对照组(55例,2018年1月至2018年6月)和观察组(62例,2018年7月至2018年12月)。对照组采用传统的骨科护理模式,观察组在对照组基础上给予FTS护理。比较两组患者护理效果。结果观察组住院时间短于对照组,住院费用低于对照组,并发症总发生率低于对照组,患者满意度高于对照组(P<0.05)。术前,两组患者髋关节Harris评分差异不显著(P>0.05);术后,两组患者髋关节Harris评分随康复时间延长明显改善(P<0.05),但组间比较,差异不显著(P>0.05)。结论 FTS理念应用于老年髋关节置换术围术期能明显缩短住院时间、降低住院费用和术后并发症的发生率,促进患者早期进行功能锻炼,尽早恢复髋关节功能,同时提高了患者对护理服务的满意度。Objective To explore the application effect of the concept of fast track surgery(FTS) in the perioperative period of hip replacement surgery in the elderly. Methods A total of 117 elderly patients who underwent hip replacement surgery in our hospital from January 2018 to December 2018 were selected as the study objects. According to the time of admission, the patients were divided into control group(55 cases, January 2018 to June 2018) and observation group(62 cases, July 2018 to December 2018). The control group adopted traditional orthopedic nursing mode, and the observation group was given FTS nursing on the basis of the control group. The nursing effect of the two groups were compared. Results The hospitalization time in the observation group was shorter than that in the control group, the hospitalization cost was lower than that in the control group, the total incidence of complications was lower than that in the control group, and the patients’ satisfaction was higher than that in the control group(P <0.05). Before operation, there was no significant difference between the two groups in the Harris score of hip joint(P>0.05);after operation, the Harris score of hip joint in the two groups improved significantly with the extension of rehabilitation time(P <0.05), but there was no significant difference between the two groups(P<0.05). Conclusion The application of FTS concept in the perioperative period of hip replacement surgery in the elderly can significantly shorten the hospitalization time, reduce the hospitalization cost and the incidence of postoperative complications, promote the early functional exercise of patients, recover the function of hip joint as soon as possible, and improve the satisfaction of patients with nursing services.
