垂体后叶素联合益母草注射液防治产后出血的效果及对氧化应激的影响

摘　　要：目的探讨垂体后叶素联合益母草注射液对产后出血患者氧化应激和炎症因子的影响。方法选取收治的产后出血高龄产妇480例为研究对象。根据治疗方法的不同将患者分为A组(n=300例)、B组(n=120例)和对照组(n=60例)。对照组在胎儿娩出后予以缩宫素治疗,B组在对照组治疗的基础上予以益母草注射液治疗,A组在B组治疗的基础上予以垂体后叶素治疗。观察3组术中、产后2 h及产后24 h产妇出血量;比较3组治疗前和治疗后以下指标:凝血功能指标[凝血活酶时间(APTT)、凝血酶原时间(PT)、纤维蛋白降解产物(FDP)、纤溶酶原激活剂抑制物-1(PAT-1)和抗凝血酶-Ⅲ(AT-Ⅲ)],氧化应激反应指标[活性氧(ROS)、丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-px)和过氧化氢酶(CAT)],炎症因子指标[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、细胞间黏附分子-1(ICAM-1)和血管内皮细胞黏附分子-1(VCAM-1)];比较3组治疗后的不良反应。结果术中、产后2 h及产后24 h产妇出血量A组<B组<对照组(P<0.05);治疗后,血清APTT、PT和FDP水平A组<B组<对照组,血清PAT-1和AT-Ⅲ水平A组>B组>对照组(P<0.05);治疗后,血清ROS和MDA水平A组<B组<对照组,血清SOD、GSH-px和CAT水平A组>B组>对照组(P<0.05);治疗后血清TNF-α、IL-6、ICAM-1和VCAM-1水平A组<B组<对照组(P<0.05)。结论垂体后叶素联合益母草注射液可有效减少剖宫产高龄产妇产后出血,改善凝血功能,缓解氧化应激,减轻炎症反应,值得临床推广应用。Objective To explore the effects of pituitrin combined with Leonurus Injections on oxidative stress and inflammatory factors in patients with postpartum hemorrhage.Methods 480 advanced age pregnant women with postpartum hemorrhage were selected as the study subjects.According to the treatment modality,the patients were divided into group A(n=300 cases),group B(n=120 cases)and control group(n=60 cases).The control group were treated with oxytocin after delivery.The group B were treated with Leonurus Injections on the basis of the control group.The group A was treated with pituitrin on the basis of the group B.The blood loss in operation,2 h postpartum and 24 h postpartum were observed among the 3 groups.The indexes of coagulation function(APTT,PT,FDP,PAT-1 and AT-Ⅲ),oxidative stress response(ROS,MDA,SOD,GSH-px and CAT)and inflammatory factors(TNF-α,IL-6,ICAM-1 and VCAM-1)were compared among the 3 groups before and after treatment.The adverse reactions after treatment were compared among the 3 groups.Results The blood loss in operation,2 h postpartum and 24 h postpartum were group A<group B<control group(P<0.05).After treatment,the serum APTT,PT and FDP levels were group A<group B<control group,while the serum PAT-1 and AT-Ⅲlevels were group A>group B>control group(P<0.05).After treatment,the serum ROS and MDA levels were group A<group B<control group,while the serum SOD,GSH-px and CAT levels were group A>group B>control group(P<0.05).After treatment,the serum levels of TNF-α,IL-6,ICAM-1 and VCAM1 were group A<group B<control group(P<0.05).Conclusion Pituitrin combined with Leonurus Injections can effectively reduce postpartum hemorrhage,improve coagulation function,alleviate oxidative stress and alleviate inflammation in advanced age pregnant women undergoing cesarean section,which is worthy of clinical application.
