摘 要:目的了解肺孤立小结节(SPN)螺旋CT三维重建在胸腔镜肺段切除术中临床价值。方法115例SPN患者进行术前CT三维重建,明确结节位置及周围血管、支气管分布,采用胸腔镜肺段切除术。结果三维重建技术对肿瘤血管显示及SPN诊断的灵敏度、特异度、准确率、阴性/阳性预测值均明显优于常规多层螺旋CT增强扫描(P<0.01或0.05)。结论术前SPN螺旋CT三维重建技术对胸腔镜肺段切除术具有重要的临床应用价值。Objective To evaluate the clinical value of preoperative spiral CT 3D reconstruction of solitary pulmonary nodules(SPN)in thoracoscopic segmentectomy.Methods Preoperative spiral CT 3D reconstruction of SPN was performed in 115 cases to determine nodule location and peripheral vascular and bronchial distribution,followed by thoracoscopic segmentectomy.Results Detection of tumor vascularity and sensitivity,specificity,accuracy,and negative/positive predictive value of SPN diagnosis in 3D reconstruction were superior to those in conventional multi-slice spiral CT enhanced scanning(P<0.01 or 0.05).Conclusion Preoperative spiral CT 3D reconstruction of SPN is valuable for thoracoscopic segmentectomy.
