产科剖宫产围手术期抗菌药物使用分析

摘　　要：目的观察和分析剖宫产围手术期预防使用抗菌药物情况。方法选取2016年1月~2018年12月本院妇产科130例产妇,所有产妇均行子宫下段横切口剖宫产手术,对产妇所用抗菌药物采用SPSS19.0软件统计分析。结果130例产妇在围手术期使用的抗菌药物类型主要有五水头孢、头孢呋辛、头孢西丁、头孢哌酮、阿奇霉素、甲硝唑和左氧氟沙星等,使用率达100.00%;130例产妇在术前应用抗菌药物共37例(28.46%),所有产妇手术持续时间均在60min左右,且失血量均低于300mL,因此术中无需二次追加抗菌药物,但在术后所有产妇都使用了抗菌药物,用药疗程最短1d,最长6d;联合用药产妇共15例,占比11.54%。联合用药以五水头孢+甲硝唑为主,占比73.33%。结论我院剖宫产围手术期抗菌药物的使用均与相关规定相符,用药较为合理,但仍存在一些不足之处,如在用药持续事件、药物联用等方面,还需要进一步加强,同时对于药物的管理和使用操作也应该进一步加强。Objective To observe and analyze the preventive use of antibiotics during the perioperative period of cesarean section.Methods 130 puerpera in the department of obstetrics and gynecology of our hospital from January 2016 to December 2018 were selected.All puerpera underwent cesarean section of the lower uterus transverse incision.The use of antibiotics of all puerpera was statistically analyzed by SPSS 19.0.Results The main types of antibiotics used in the perioperative period in 130 puerpera were pentahydrate cephalosporin,cefuroxime,cefoxitin,cefoperazone,a zithromycin,metronidazole and levofloxacin,with the use rate of 100.00%.A total of 37 cases(28.46%)of 130 puerpera were given antibiotics before surgery.The duration of surgeries of all puerpera was about 60 min and the blood loss was less than 300ml,so there was no need to add antibiotics.However,all puerpera were treated with antibiotics after operation,with the shortest course of treatment of 1day and the longest course of treatment of 6 days.A total of 15 cases(11.54%)were given combined treatment.The combination drugs were mainly pentahydrate cephalosporin combined with metronidazole,accounting for 73.33%.Conclusion The use of antibiotics in the perioperative period of cesarean section in our hospital is consistent with relevant regulations and the use of antibiotics is relatively reasonable.However,there are still some deficiencies such as continuous drug use,drug combination and other aspects,which need to be strengthened further.In addition,the management and use of drugs should also be further strengthened.
