摘　　要：目的观察糖皮质激素联合静脉丙种球蛋白治疗结缔组织病相关间质性肺疾病的疗效。方法收集本院2016年2月~2019年2月结缔组织病相关间质性肺疾病患者60例,随机分成治疗组和对照组各30例。两组均予抗炎等基础性治疗,注意保持水电解质平衡。对照组予甲基泼尼松龙注射液,治疗组同时联合静脉丙种球蛋白,治疗后对比分析两组的临床疗效。结果治疗后总有效率治疗组为96.67%,对照组为70.00%,两组临床疗效对比差异有统计学意义(P<0.05)。治疗组和对照组患者治疗前的抗SSA抗体、抗SSB抗体、抗SM抗体及抗J0-1抗体及抗ro-52抗体比较,差异无统计学意义(P>0.05),治疗后,两组患者的上述抗体均较治疗前显著降低,差异有统计学意义(P<0.05);且治疗组治疗后显著低于对照组治疗后,组间比较差异具有统计学意义(P<0.05)。结论糖皮质激素联合静脉丙种球蛋白治疗早期结缔组织病相关间质性肺疾病疗效显著,值得临床广泛推广和应用。Objective To observe curative effects of glucocorticoid combined with intravenous gamma globulin in treatment of interstitial lung diseases associated with connective tissue disease.Methods Sixty patients with connective tissue disease-related interstitial lung diseases in our hospital from February 2016 to February 2019 were collected.They were randomly divided into the treatment group and the control group,with 30 cases in each group.Patients in two groups were all given basic treatment such as anti-inflammatory therapy,and attention was paid to maintaining water and electrolyte balance.The control group was given methylprednisolone injection and the treatment group was given methylprednisolone injection combined with intravenous gamma globulin.The clinical curative effects of the two groups were compared after treatment.Results The total effective rate after treatment was 96.67%in the treatment group and 70.00%in the control group.There was significant difference in clinical curative effects between the two groups(P<0.05).There was no statistically significant difference in anti-SSA antibody,anti-SSB antibody,anti-SM antibody,anti-J0-1 antibody and antiro-52 antibody between the treatment group and the control group before treatment(P>0.05).After treatment,the above antibodies were significantly lower in the two groups than before treatment,and the difference was statistically significant(P<0.05).Moreover,the treatment group was significantly lower than the control group after treatment,and the difference between the groups was statistically significant(P<0.05).Conclusion Glucocorticoid combined with intravenous gamma globulin in treatment of interstitial lung diseases associated with connective tissue disease has significant curative effects,which is worthy of wide promotion and application in clinical practice.
