护士分级监控管理模式对脑卒中偏瘫患者肢体功能恢复及生活质量的影响

摘　　要：目的观察护士分级监控管理模式的应用对脑卒中(CVA)偏瘫患者肢体功能恢复及生活质量的影响。方法将广州市第十二人民医院神经内科分级监控管理模式实施前(2018年1~7月)的63例CVA偏瘫患者设为对照组,分级监控管理模式实施后(2018年8月~2019年3月)的65例CVA偏瘫患者设为研究组。比较两组入组时、护理3个月后肢体功能恢复及生活质量。结果两组入组时的肌力分级比较,差异无统计学意义(P>0.05);两组护理3个月后肌力分级均优于本组入组时,差异有统计学意义(P<0.05);研究组护理3个月后肌力分级优于对照组,差异有统计学意义(P<0.05)。护理3个月后,两组生理健康领域中总体健康、生理职能、生理功能3个维度评分均明显高于本组入组时,心理健康领域中活力评分明显高于本组入组时(P<0.05)。研究组护理3个月后生理健康领域中总体健康、生理职能、生理功能,心理健康领域中活力评分明显高于对照组,差异有统计学意义(P<0.05)。结论护士分级监控管理模式可确保各项护理措施的落实,提高护理效果,可促进CVA偏瘫患者肢体功能恢复,提高生活质量。Objective To observe the application nurses′hierarchical monitoring and management mode on the recovery of limb function and quality of life in stroke(CVA)patients with hemiplegia.Methods Guangzhou 12th People′s Hospital before the implementation of monitoring management mode(from January to July 2018)of 63 patients with CVA hemiplegia were set as control group,classification management mode after implementation(from August 2018 to March 2019)of 65 patients with CVA hemiplegia were set as study group,in the two groups patients into the group,nursing after 3 months limb function recovery and quality of life were compared.Results There was no statistically significant difference in muscle strength grading between the two groups(P>0.05).After 3 months of nursing care,the muscle strength grade of the two groups was better than that into the group,and the difference was statistically significant(P<0.05).The muscle strength grade of the study group was better than that of the control group after 3 months of nursing,and the difference was statistically significant(P<0.05).After 3 months of nursing,the scores of overall health,physiological function and physiological function in the field of physical health in the two groups were significantly high than those into the group,and the scores of vitality in the field of mental health were significantly high than those into the group(P<0.05).The overall health,physiological function and physiological function of the study group were significantly higher than those of the control group(P<0.05).Conclusion The nurses′hierarchical monitoring management mode can ensure the implementation of various nursing measures,improve the effect of care.It can promote the recovery of limb function and improve the quality of life in CVA patients with hemiplegia.
