摘 要:目的:比较不同浓度氯化钠溶液氧气驱动雾化吸入对新生儿肺炎的雾化效果。方法:将新生儿120例随机分为0.9%组、3%组、5%组、7%组各30例。分别给予0.9%、3%、5%、7%氯化钠液5 mL+盐酸氨溴索注射液15 mg配制成雾化液,采用氧气驱动雾化装置连接面罩吸入给药。结果:0.9%组PaO2、PCO2、HCO2^-血气指标均明显低于3%组、5%组、7%组,差异有统计学意义(P<0.05);3%、5%、7%组肺部感染评分明显低于0.9%组,差异有统计学意义(P<0.05);3%组雾化适度明显优于0.9%组、5%组、7%组,差异有统计学意义(P<0.05);3%组、5%组、7%组雾化吸入治疗的新生儿喘息缓解时间、肺部炎症消失时间短于0.9%组,5%组、7%组,咳嗽缓解时间长于0.9%组、3%组,差异均有统计学意义(P<0.05)。结论:3%、5%、7%高浓度氯化钠溶液雾化吸入治疗新生儿肺炎,能较快地改善患儿的血气指标,降低肺部感染。Objective:To compared the effect of oxygen-driven atomization inhalation with different concentration of sodium chloride solution on neonatal pneumonia.Method:120 newborns were randomly divided into the 0.9%,3%,5% and 7% groups with 30 cases in each group,5 mL of 0.9%,3%,5% and 7% sodium chloride solution and 15 mg of ambroxol hydrochloride injection were administered respectively to prepare aerosol solution,and oxygen-driven atomizing device was used to connect the mask inhalation for administration.Results:PaO2,PCO2 and HCO2^-blood gas indexes in the 0.9%group were significantly lower than those in the 3%,the 5% and the 7% group,the difference was statistically significant(P<0.05).The scores of pulmonary infection in the 3%,the 5% and the 7% sodium chloride group were significantly lower than those in the 0.9%group,the difference was statistically significant(P<0.05).The moderate atomization rate in the 3% group was significantly better than that in the 0.9%,the 5%and the 7%groups,the difference was statistically significant(P<0.05).The neonatal asthma relief time and pulmonary inflammation disappearance time in the 3%,the 5% and the 7% group were shorter than the 0.9% group,but cough relief time in the 5% and the 7% groups was longer than the 0.9% and the 3% groups,the difference was statistically significant(P<0.05).Conclusion:3%,5%,7% high concentration sodium chloride solution oxygen-driven atomization inhalation treatment of neonatal pneumonia can quickly improve the blood gas indicators of children,reduce pulmonary infection,in particular.
