摘  要:目的评估三维双激发平衡式稳态自由进动序列(3D-FIESTA-C)显示下腰痛患者腰椎脊神经与椎间盘的图像质量.方法对40例下腰痛患者行腰椎MRI常规序列[矢状位迭代最小二乘估算法水脂分离技术(IDEAL),矢状位T1WI,横断位T2WI],矢状位3D-FIESTA-C序列扫描,对3D-FIESTA-C原始图像进行MPR后用于进一步分析.由2名医师独立对3D-FIESTA-C和常规序列脊神经-椎间盘的显示进行主观评分,并分析3D-FIESTA-C、IDEAL、T2WI中脊神经和脑脊液的SNR以及二者的CNR.结果3D-FIESTA-C主观评分(4.77±0.42)高于常规序列主观评分(3.00±0.32),差异有统计学意义(P<0.01).对于3D-FIESTA-C、IDEAL、T2WI,脊神经的SNR分别为33.07±12.29、39.91±18.16、27.40±10.34,3D-FIESTA-C与IDEAL间无统计学差异,但优于T2WI(P<0.05);脑脊液的SNR分别为269.15±70.30、213.13±40.52、119.25±35.15,3D-FIESTA-C优于后二者(P=0.000);脊神经和脑脊液的CNR分别为235.92±62.04、172.69±38.75、91.85±30.62,3D-FIESTA-C优于后二者(P=0.000).结论3D-FIESTA-C序列能清晰显示脊神经的形态、走行、脊神经与椎间盘的相对空间关系,可作为下腰痛患者常规腰椎MR序列的重要补充.Objective To evaluate image quality of 3D-FIESTA-C sequence displaying lumbar spinal nerves and intervertebral discs in patients with low back pain.Methods Routine protocol for lumbar MRI examinations(sagittal IDEAL,sagittal T1WI and axial T2WI sequences)and sagittal 3D-FIESTA-C sequence were performed in 40 patients with low back pain.The MPR of the 3D-FIESTA-C images were used for further analysis.Two radiologists scored the different sequences of images independently.The SNR and the CNR between spinal nerves and cerebrospinal fluid were calculated at 3D-FIESTA-C,IDEAL,T2WI sequences.Results The image scores of 3D-FIESTA-C(4.77±0.42)was higher than that of the routine protocol(3.00±0.32),and the difference was statistically significant(P<0.01).For 3D-FIESTA-C,IDEAL and T2 WI sequences,the SNR of the spinal nerves was 33.07±12.29,39.91±18.16,27.40±10.34,respectively,and there was no statistical difference between 3D-FIESTA-C and IDEAL,3D-FIESTA-C was better than T2 WI(P<0.05).The SNR of cerebrospinal fluid was 269.15±70.30,213.13±40.52,119.25±35.15,respectively,and 3D-FIESTA-C was better than IDEAL,T2WI sequences(P=0.000).The CNR between spinal nerves and cerebrospinal fluid was 235.92±62.04,172.69±38.75,91.85±30.62,respectively,and 3D-FIESTA-C was better than IDEAL,T2WI sequences(P=0.000).Conclusion The 3D-FIESTA-C sequence could display spinal nerves and the relative spatial relationship between the spinal nerves and the intervertebral discs,and addition of a 3D-FIESTA-C sequence to a routine lumbar MR protocol may be useful in patients with low back pain.
