摘 要:目的探讨南宁城区轻型地中海贫血(地贫)孕妇甲状腺功能各项检测指标的变化。方法选取2017年7月至2018年7月在广西医科大学第一附属医院产科门诊建档进行产检并确诊为轻型地贫的孕妇238例为地贫组,选取同期健康孕妇448例为正常组。检测两组孕妇抗甲状腺过氧化物酶抗体(TPOAb)及孕早、中、晚期促甲状腺素(TSH)和游离甲状腺激素(FT4),并进行比较分析;比较两组甲状腺功能异常的患病率。结果两组孕妇TPOAb阳性率比较差异无统计学意义(χ^2=1.128,P>0.05)。孕早期两组的TSH(t=0.834)、FT4(t=1.194)及甲状腺功能异常患病率(χ^2=0.230)比较差异均无统计学意义(均P>0.05);孕中期地贫组的TSH(t=-1.181)、FT4(t=0.181)与正常组比较差异均无统计学意义(均P>0.05),两组甲状腺功能异常患病率无显著性差异(χ^2=0.000,P>0.05);孕晚期两组TSH(t=-2.702)、FT4(t=0.119)及甲状腺功能异常患病率(χ^2=1.557)比较差异均无统计学意义(均P>0.05)。结论在南宁城区,轻型地中海贫血患者孕期甲状腺功能与同期健康孕妇无显著性差异。Objective To investigate the changes of thyroid function in patients with mild thalassemia in the urban area of Nanning,Guangxi.Methods 238 pregnant women with mild thalassemia who were registered to come to the Clinical Department of Obstetrics of our hospital for regular pregnancy health checkup during the period from July 2017 to July 2018 were selected as thalassemia group and the other 448 healthy pregnant women in the same period were taken as the normal group.Thyrotropin(TSH),free thyroid hormone(FT4) and anti-thyroid peroxidase antibody(TPOAb) of the three trimesters were measured and compared between the two groups.The prevalence of thyroid dysfunction was compared between the two groups.Results There was no significant difference in TPOAb positive rate between the two groups(χ^2=1.128,P>0.05).There was no marked difference between the two groups in TSH(t=0.834),FT4(t=1.194) and rate of thyroid functional disturbance(χ^2=0.230,P>0.05)during the first trimester.There was no significant difference in TSH(t=-1.181),FT4(t=0.181) between the two groups during the second trimester,and the prevalence of thyroid dysfunction of the two groups showed no difference(χ^2=0.000,P>0.05).The TSH(t=-2.702),FT4(t=0.119) and the rate of thyroid dysfunction(χ^2=1.557) also showed no statistical differences(P>0.05) during the third trimester.Conclusion There was no considerable difference in thyroid function between the pregnant women with mild thalassemia and healthy pregnant women.
