含多西他赛联合化疗对癌症病人胰岛β细胞功能影响

摘　　要：目的观察胰岛功能正常的恶性肿瘤病人接受以多西他赛为基础的联合方案化疗后,胰岛β细胞功能的变化。方法糖代谢正常的恶性肿瘤病人52例,应用以多西他赛为基础的联合方案化疗,观察化疗期间以及化疗结束后1月病人空腹血糖、空腹血胰岛素(FINS)、胰岛素抵抗指数、C肽释放试验的变化。结果接受4周期化疗后,病人空腹血糖、FINS以及胰岛素抵抗指数较化疗前及化疗后1个月升高,差异有统计学意义(F=8.041~10.486,P<0.05);C肽释放试验异常23例。化疗结束后1个月,病人空腹血糖、FINS以及胰岛素抵抗指数与化疗前比较差异无统计学意义(P>0.05),C肽释放试验仍有9例未恢复正常。结论以多西他赛为基础的联合化疗可损伤胰岛β细胞,且影响血糖代谢。Objective To investigate the change in the function of pancreaticβcells after docetaxel-based combined chemotherapy in patients with malignant tumor who have normal pancreatic islet function before chemotherapy.Methods A total of 52 patients with malignant tumors and normal glucose metabolism were enrolled and treated with docetaxel-based combined chemotherapy.Fasting blood glucose,fasting insulin,insulin resistance index,and C-peptide release test results were observed during chemotherapy and at 1 month after chemotherapy.Results Fasting blood glucose,fasting insulin,and insulin resistance index after 4 cycles of chemotherapy were significantly higher than those before chemotherapy and at 1 month after chemotherapy(F=8.041-10.486,P<0.05),and 23 patients had abnormal results of the C-peptide release test.There were no significant changes in fasting blood glucose,fasting insulin,and insulin resistance index from before chemotherapy to 1 month after chemotherapy(P>0.05),and 9 patients still had abnormal results of the C-peptide release test.Conclusion Docetaxel-based combined che-motherapy can damage pancreaticβcells and affect blood glucose metabolism.
