摘 要:目的:探讨外周血中内皮细胞源微粒(EMPs)水平与冠状动脉(冠脉)病变严重程度的相关性,分析EMPs与急性心肌梗死(AMI)发生之间的关联。方法:将研究对象分为非冠心病组(Non-CHD组,57例)和AMI组(110例)。根据血管病变支数,将AMI组患者分为单支血管病变亚组、双支血管病变亚组和3支血管病变亚组。评估AMI组患者TIMI危险评分,将其分为低危亚组、中危亚组和高危亚组。采用流式细胞仪检测患者外周静脉血EMPs水平,比较各组EMPs水平差异。采用多重线性回归分析EMPs与部分临床指标的相关性。结果:AMI组EMPs水平显著高于Non-CHD组,差异具有统计学意义[(43.87±21.53)%∶(32.16±20.10)%,P=0.001];AMI单支血管病变亚组、双支血管病变亚组和3支血管病变亚组EMPs水平分别为(35.23±22.17)%、(42.98±17.46)%、(49.19±21.52)%,组间差异有统计学意义(P=0.018)。TIMI评分低危亚组、中危亚组和高危亚组EMPs水平分别为(28.22±5.38)%、(45.98±19.98)%、(52.56±18.03)%,组间差异有统计学意义(P=0.001)。多重线性回归分析显示,血清LDL-C水平、LVEF与EMPs水平之间具有线性相关关系。结论:AMI患者外周血EMPs水平升高,EMPs水平与AMI冠脉病变程度和预后风险相关。Objective:To discusse the correlation between the level of endothelial cell-derived microparticles(EMPs)in peripheral blood and the severity of coronary artery disease and to analyze the association between EMPs and the occurrence of acute myocardial infarction(AMI).Method:The subjects were divided into Non-CHD group(n=57)and AMI group(n=110).According to the number of lesion vessels,AMI patients were divided into single lesion vessel subgroup,double lesion vessels subgroup and three lesion vessels subgroup.According to the TIMI score,AMI patients were divided into low-risk subgroup,medium-risk subgroup and high-risk subgroup.The levels of EMPs in peripheral blood were measured by flow cytometry,and the differences of EMPs in each group were compared.Multiple linear regression analysis was used to analyze the correlation between EMPs and clinical indicators.Result:The level of EMPs in AMI group was significantly higher than that in Non-CHD group[(43.87±21?53)%vs(32.16±20.10)%,P=0.001].The levels of EMPs in single lesion vessel subgroup,double lesion vessels subgroup,and three lesion vessels subgroups were(35.23±22.17)%,(42.98±17.46)%,and(49.19±21.52)%,respectively,the difference among three groups was statistically significant(P=0.018).The levels of EMPs in low-risk subgroup,middle-risk subgroup,and high-risk subgroup were(28.22±5.38)%,(45.98±19.98)%,and(52.56±18.03)%,there was significant difference among three groups(P=0.001).Multiple linear regression analysis showed that there was a linear correlation between serum LDL-C level,LVEF and EMPs level.Conclusion:The level of EMPs in peripheral blood in patients with AMI is increased and correlated with the severity of coronary lesions and prognosis of AMI.
