摘 要:目的探讨循环内皮(CECs)细胞计数在心力衰竭患者左心室功能及预后评价中的临床应用。方法随机选取2015年5月-2017年5月某医院心内科收治的心力衰竭患者40例,将这些患者作为心力衰竭组,另选取同期该院接收的健康体检人员40例作为对照组,采用美国ecton&Dickinson公司生产的流式细胞仪对2组人员的CECs细胞数进行测量,并采用美国ATL-4型LOGIQ400多普勒实时超声诊断仪对2组人员进行超声检查,然后对2组人员的CECs、左室收缩末期容积指数(LVESVI)、左室舒张末期容积指数(LVEDVI)、左室射血分数(LVEF)、左室心肌质量指数(LVMI)等左心室功能指标、心力衰竭组患者的主要不良事件发生情况进行统计分析。结果心力衰竭组患者的CECs多于对照组(P<0.05),LVESVI、LVEDVI、LVMI均高于对照组(P<0.05),LVEF低于对照组(P<0.05),CECs≥9 663个/mL患者的主要不良事件发生率66.7%(10/15)高于CECs<9 663个/mL患者的28.0%(7/25)(P<0.05)。结论循环内皮细胞计数能够有效评价心力衰竭患者左心室功能及预后,值得临床充分重视。Objective To investigate the clinical application of circulating endothelial cells(CECs) in evaluation of the left ventricular function and prognosis of patients with heart failure. Methods Forty patients with heart failure were randomly selected intoa heart failure groupin a hospital from May 2015 to May 2017, and during the same period, 40 healthy persons who received physical examinations in the hospital were selected as a control group. The CECs were detected by flow cytometry(ecton&Dickinson companies in the United States)in two groups, and all the persons received the ultrasound diagnostic examinations by the ATL-4 LOGIQ400 real-time ultrasound dopplerinstrument. Then the CECs, left ventricular function indexes;such as the left ventricular end-systolic volume index(LVESVI), left ventricular end-diastolic volume index(LVEDVI), left ventricular ejection fraction(LVEF), left ventricular myocardial mass index(LVMI) in the two groups, and the major adverse events in the heart failure group wereinvestigated and the results were statistically analyzed. Results The CECs, LVESVI, LVEDVI and LVMI in the heart failure group were significantly higher than those in the control group(P<0.05). LVEF in the heart failure group was significantly lower than that in the control group(P<0.05). In addition, in the heart failure group, the incidence of major adverse events in the CECs ≥9663/ml patients was 66.7%(10/15), which was significantly higher than that in the CECs<9663/ml patients(7/25, 28.0%)(P<0.05). Conclusion The CECs can effectively evaluate the left ventricular function and prognosis of patients with heart failure.
