归脾汤加减治疗慢性心力衰竭合并贫血气虚血瘀证30例临床观察

摘　　要：目的:观察归脾汤加减治疗慢性心力衰竭合并贫血气虚血瘀证的临床疗效。方法:将60例慢性心力衰竭合并贫血气虚血瘀证的患者随机分为治疗组和对照组,每组各30例。对照组予以常规抗心衰治疗,治疗组在对照组治疗的基础上予以归脾汤加减治疗。观察2组患者临床疗效、N端前脑钠肽(NT-pro BNP)、血红蛋白(Hb)、6min步行距离(6MWD)及超声心动图测量左室射血分数(Lv EF)的改善情况。结果:总有效率治疗组为93.33%,高于对照组的73.33%,差异有统计学意义(P<0.05);2组患者治疗后NT-pro BNP、Hb、6MWD、Lv EF均得到改善,且治疗组改善更明显,差异有统计学意义(P<0.05)。结论:归脾汤加减能改善慢性心力衰竭合并贫血气虚血瘀证患者的心功能和临床症状,提高患者运动耐量,并在一定程度上纠正贫血,但其作用机制仍待进一步研究。Objective:To investigate the clinical effect of modified Guipi decoction in the treatment of chronic heart failure with anemia and Qi deficiency and blood stasis syndrome.Methods:A total of 60 patients with chronic heart failure with anemia and Qi deficiency and blood stasis syndrome were randomly divided into treatment group and control group,with 30 patients in each group.The patients in the control group were given conventional cardiotonic treatment,and those in the treatment group were given modified Guipi decoction in addition to the treatment in the control group.The two groups were compared in terms of clinical outcome,N-terminal pro-brain natriuretic peptide(NT-proBNP),hemoglobin(Hb),6-minute walk distance(6 MWD),and left ventricular ejection fraction(LvEF)on echocardiography.Results:The treatment group had a significantly higher overall response rate than the control group(93.33%vs 73.33%,P<0.05).Both groups had significant improvements in NT-proBNP,Hb,6 MWD,and LvEF after treatment,and the treatment group had significantly greater improvements than the control group(P<0.05).Conclusion:In patients with chronic heart failure with anemia and Qi deficiency and blood stasis syndrome,modified Guipi decoction can improve their cardiac function,clinical symptoms,and exercise tolerance and correct anemia to a certain degree,but further studies are needed to clarify its mechanism of action.
