腹腔镜疝囊高位结扎治疗小儿腹股沟疝36例临床观察

摘　　要：目的探讨腹腔镜疝囊高位结扎治疗小儿腹股沟疝36例临床效果。方法选取2017年6月—2019年5月江西中医药高等专科学校(附属医院)收治的小儿腹股沟疝患儿72例,按照随机数字表法分为对照组和观察组,每组各36例。对照组患儿采用传统切开疝囊高位结扎术治疗,观察组患儿采用腹腔镜疝囊高位结扎术治疗,观察分析两组患儿的术中出血量、手术时间、住院时间、术后并发症(阴囊肿胀、腹胀、感染、对侧隐性疝发现状况)及术后复发状况。结果观察组患儿的术中出血量少于对照组患儿,手术时间及住院时间均显著短于对照组患儿,差异有统计学意义(P<0.05)。观察组患儿出现的并发症现象显著低于对照组患儿,差异有统计学意义(P<0.05)。两组患儿术后复发率对比,差异无统计学意义(P>0.05)。结论采用腹腔镜疝囊高位结扎对小儿腹股沟疝的治疗效果显著,能够显著降低术中出血量等临床指标,且术后出现的并发症现象较少,安全性较高。Objective To investigate the clinical effect of laparoscopic high ligation of hernia sac in 36 children with inguinal hernia.Methods Seventy-two children with inguinal hernia admitted to Jiangxi College of Traditional Chinese Medicine(Affiliated Hospital)from June 2017 to May 2019 were randomly divided into control group and observation group,36 cases in each group.The control group was treated with traditional incision hernia sac high ligation,while the observation group was treated with laparoscopic hernia sac high ligation.The bleeding volume,operation time,hospitalization time,postoperative complications(swelling of scrotum,abdominal distention,infection,detection of contralateral recessive hernia)and recurrence after operation were observed and analyzed.Condition.Results The amount of intraoperative bleeding in the observation group was less than that in the control group,and the operation time and hospitalization time were significantly shorter than those in the control group(P<0.05).The complications in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the recurrence rate between the two groups(P>0.05).Conclusion Laparoscopic high ligation of hernia sac is effective in the treatment of inguinal hernia in children.It can significantly reduce the amount of bleeding during operation and other clinical indicators,and has fewer complications and higher safety.
