摘　　要：目的观察不同血流速度血液灌流(HP)联合血液透析滤过(HDF)对尿毒症患者甲状旁腺素(PTH)的清除效果。方法选取2017年3月至6月江苏省人民医院血液净化中心收治的31例尿毒症维持性血液透析患者进行自身对照,每人行两次HDF+HP联合治疗,血流速度分别为250 mL/min(观察组)和200 mL/min(对照组)。比较两组治疗后血PTH水平变化,观察连接管路及透析器的凝血情况。结果两组患者治疗后血PTH水平比较,差异有统计学意义(P<0.05)。对照组有7例(16.2%)连接管路滤网发生凝血,观察组均未发生凝血。结论采用250和200 mL/min血流速度进行HDF+HP治疗对尿毒症患者PTH的清除效果不同,250 mL/min效果更好。Objective To observe the effect of hemoperfusion(HP)combined with hemodiafiltration(HDF)with different velocities of blood flow on the clearance of parathyroid hormone(PTH)in patients with uremia.Methods Thirty-one uremia patients with maintained hemodialysis admitted to the First Affiliated Hospital of Nanjing Medical University from March to June 2017 were treated with HP combined with HDF,twice per patient,with a blood flow rate of 250 mL/min(as the observation group)and 200 mL/min(as the control group).The changes of blood PTH level after treatment were compared between the two groups,and the coagulation of the connecting pipeline and dialyzer was observed.Results After treatment,there was significant difference in PTH level between the two groups(P<0.05).In control group,7 cases(16.2%)occurred coagulation of the connecting pipeline,and no patient occurred coagulation in the observation group.Conclusion HDF+HP treatment with 250 and 200 mL/min blood flow velocity has different effect on PTH clearance in uremia patients,and the effect of 250 mL/min is better.
