输尿管镜取石术治疗输尿管结石所致顽固性肾绞痛的临床疗效

摘  要:目的观察输尿管镜取石术处理输尿管结石所致顽固性肾绞痛的临床效果。方法选取贵州省福泉市第一人民医院2016年11月至2017年11月收治的输尿管结石所致顽固性肾绞痛患者88例,随机分为两组,每组44例。对照组行输尿管镜下气压弹道碎石术治疗,观察组行输尿管镜下钬激光碎石术治疗。观察两组手术情况、治疗有效率及并发症、复发率。结果观察组出血量(10.52±1.36)mL、住院时间(3.10±0.07)d、术后疼痛(0.22±0.03)分,均显著少于对照组[(26.79±2.58)mL、(6.78±0.42)d、(3.17±0.48)分],观察组一期取净率97.73%、有效率97.73%,显著高于对照组(70.45%、70.45%),并发症发生率2.27%、复发率2.27%,显著低于对照组(9.09%、20.45%),两组数据差异显著(P<0.05)。结论输尿管镜下钬激光碎石术用于治疗输尿管结石所致顽固性肾绞痛,具有出血量少、恢复速度快的优势。Objective To observe the clinical effect of ureteroscopic lithotomy in the treatment of refractory renal colic caused by ureteral calculi.Methods From November 2016 to November 2017,88 patients with refractory renal colic caused by ureteral calculi were randomly divided into two groups(44 in each group).The control group was treated with ureteroscopic lithotomy,while the observation group was treated with ureteroscopic lithotomy.The operative condition,effective rate,complication and recurrence rate of the two groups were observed.Results Blood loss(10.52±1.36 mL),hospitalization time(3.10±0.07 d),postoperative pain(0.22±0.03)in observation group were significantly lower than those in the control group(26.79±2.58 mL,6.78±0.42 d,3.17±0.48).The incidence of complications was 2.27% and the recurrence rate was 2.27%,which were significantly lower than that of the control group(9.09%,20.45%).There were significant differences between the two groups in the above indicators(P<0.05).Conclusion Ureteroscopic lithotomy for the treatment of refractory renal colic caused by ureteral calculi has the advantages of less blood loss and faster recovery.
