摘　　要：目的分析不同时机行有创机械通气治疗慢阻肺急性加重合并重度呼吸衰竭的临床效果。方法选取2017年10月至2018年12月滨州医学院附属医院收治的73例患者展开研究,对照治疗时间差异分为对照(6 h后)和观察组(6 h前);对比分析临床指标用时、动脉血气和肺功能改善情况。结果观察组有创通气时间(60.36±15.23)h,总机械通气时间(158.26±25.23)h,ICU住院时间(7.23±1.45)d,临床各指标明显较对照组短,(P<0.05)。观察组心率较稳定,动脉血气各指标明显较对照组好,肺功能各指标明显较对照组高,(P<0.05)。结论对慢阻肺急性加重合并重度呼吸衰竭患者尽早实施有创机械通气治疗疗效显著。Objective To analyze the clinical effect of invasive mechanical ventilation at different times in the treatment of acute exacerbation of copd with severe respiratory failure.Methods A total of 73 patients admitted to our hospital from October 2017 to December 2018 were selected for the study.The clinical time,arterial blood gas and lung function were compared and analyzed.Results The duration of invasive ventilation(60.36±15.23)h,total mechanical ventilation(158.2625.23)h,length of stay in ICU(7.23±1.45)d in the observation group were significantly shorter than those in the control group(P<0.05).The heart rate of the observation group was relatively stable,and the indicators of arterial blood gas were significantly better than those of the control group,and the indicators of lung function were significantly higher than those of the control group(P<0.05).Conclusion Early invasive mechanical ventilation for patients with acute exacerbation of copd combined with severe respiratory failure is effective.
