唐山市婴幼儿先天性心脏病危险因素的病例对照研究

摘　　要：目的探讨父母围孕期危险因素暴露与婴幼儿先天性心脏病的发病风险.方法选择我院2014年1月至2017年12月住院手术的先天性心脏病息儿为病例组,对照组为同期经临床及超声心动图检查除外先天性心脏病的婴幼儿.对病例组及对照组婴幼儿父母一般情况及围孕期暴露情况进行问卷调查.结果我院282例先天性心脏病婴幼儿与362例非先天性心脏病婴幼儿的父母一般情况及围孕期暴露的相关危险因素分析显示:母亲孕早期用药(OR=4.63,P<0.05)、病毒感染(OR=3.12,P<0.05)、接触有机溶剂(OR=1.87,P<0.05)、烫染发(OR=2.19,P<0.05)和工作环境暴露(OR=2.85,P<0.05)均是造成先天性心脏病的危险因素.结论婴幼儿父母围孕期的危险因素的暴露与先天性心脏病的发生存在相关性.Objective To investigate the risk of congenital heart disease in infants and children with perinatal exposure to risk factors in Tangshan City.Methods Infants and children with congenital heart disease who were hospitalized and operated in our hospital from January 2014 to December 2017 were selected as the case group and the control group contained infants and young children without congenital heart disease confirmed by clinical and cardiac ultrasound examination during the same period.A questionnaire survey was conducted for the general situation of parents of infants and children of the two groups as well as the perinatal exposure to the risk factors.Results Analysis of the general situation of parents of both two groups and the related risk factors of prenatal exposure showed that medication(OR=4.63,P<0.05),viral infection(OR=3.12,P<0.05),exposure to organic solvents(OR=1.87,P<0.05),hair perm and dyeing(OR=2.19,P<0.05)and work environment exposure(OR=2.85,P<0.05)were all risk factors of congenital heart disease.Conclusion The infant parents exposure to the risk factors in perinatal period is correlated with the occurrence of congenital heart disease.
