摘　　要：目的分析2型糖尿病合并H型高血压患者与尿酸(UA)及反映血管病变情况指标(ABI、TBI、baPWV、颈动脉病变情况)的相关性。方法选取北京丰台医院收治的2型糖尿病(T2DM)并高血压病患者120例,根据是否合并H型高血压分为2型糖尿病合并H型高血压组(观察组)60例,2型糖尿病合并非H型高血压组(对照组)60例。检测血同型半胱氨酸(Hcy),尿酸(UA),甘油三酯(TG),胆固醇(TC),高密度脂蛋白(HDL),低密度脂蛋白(LDL),空腹血糖(FPG)、糖化血红蛋白(HBA1c),记录血压(SBP)值。测量研究对象踝臂指数(ABI)、趾臂指数(TBI)、肱踝脉搏波速度(baPWV)。行颈动脉B超了解颈动脉斑块发生率。进行组间比较。结果观察组的UA、baPWV、颈动脉斑块发生率高于对照组(P<0.05),观察组的ABI低于对照组(P>0.05),观察组的TBI低于对照组(P<0.05)。结论2型糖尿病合并H型高血压患者血Hcy与ABI、TBI、baPWV、UA相关,通过无创性检测2型糖尿病合并高血压病患者的Hcy、UA、ABI、TBI、baPWV和颈动脉超声,为早期采取相应的干预措施提供依据,在降压、降糖的同时关注对血浆同型半胱氨酸、尿酸及动脉硬化的治疗亦有一定价值。Objective To analyze the correlation between type 2 diabetes mellitus patients with H hypertension and uric acid(UA)and indicators reflecting vascular lesions(ABI,TBI,baPWV,carotid artery lesions).Methods 120 patients with type 2 diabetes mellitus(T2DM)and hypertension admitted to Fengtai Hospital of Beijing were divided into two groups:type 2 diabetes mellitus with H hypertension group(observation group)and type 2 diabetes mellitus with non-H hypertension group(control group).Blood homocysteine(Hcy),uric acid(UA),triglyceride(TG),cholesterol(TC),high density lipoprotein(HDL),low density lipoprotein(LDL),fasting blood glucose(FPG),glycosylated hemoglobin(HBA1c)were measured and blood pressure(SBP)was recorded.The ankle-brachial index(ABI),toe-brachial index(TBI)and brachial-ankle pulse wave velocity(baPWV)were measured.The incidence of carotid plaque was assessed by B-mode ultrasonography.Intergroup comparison was made.Results The incidence of UA,baPWV and carotid plaque in the observation group was higher than that in the control group(P<0.05),the ABI in observation group was lower than that in control group(P>0.05),and the TBI in observation group was lower than that in control group(P<0.05).Conclusion Blood Hcy in type 2 diabetes mellitus patients with H-type hypertension is correlated with ABI,TBI,baPWV and UA.Non-invasive detection of Hcy,UA,ABI,TBI,baPWV and carotid ultrasound in type 2 diabetes mellitus patients with hypertension provides a basis for early intervention.It is also valuable to treat plasma homocysteine,uric acid and arteriosclerosis while reducing blood pressure and blood glucose.
