摘  要:目的对北京口岸1例自愿检测的境外输入性疟疾病例进行综合检测分析,为国际旅行者输入性疟疾快速准确检测提供技术参考。方法采用镜检法、实时荧光PCR、普通PCR、巢式PCR法检测疟原虫,同时进行血常规和血生化检测,并对检测结果进行综合分析。结果首先用实时荧光PCR法检出疟原虫,镜检法未检出,普通PCR和巢式PCR法复检确认为间日疟原虫,疟原虫密度为4 686拷贝/μl。血常规和血生化检测异常指标有:红细胞、血红蛋白、红细胞压积和血小板不同程度降低,C反应蛋白、乳酸脱氢酶、总胆红素、直接胆红素和肌酐显著升高。结论本病例确认为间日疟原虫感染,疟原虫核酸检测结合血常规和血生化指标分析,有助于提高国际旅行者输入性疟疾检测水平。Objective To analyze the comprehensive detection of an imported malaria in an voluntary testing case at Beijing port,to provide technical reference for rapid and accurate detection of imported malaria among the interna-tional travelers. Methods Several methods including bloodsmear microscopy,Real-time PCR,regular PCR and nested PCR were conducted to confirm the infection,and blood routine and biochemistry were also tested. Results Plasmodium was firstly detected by Real-time PCR,microscopy detection showed a negative result. Plasmodium vivax was confirmed by regular PCR and nested PCR. Plasmodium density was 4 686 copies/μl. The abnormal index included the reduced index of blood routine(RBC,Hb,HCT and PLT) and the elevated index of blood biochemistry(CRP,LDH,TBIL,DBIL and CREA). Conclusion The imported malaria case was confirmed as P. vivax infection. The Plasmodium detection and blood routine,biochemistry detection could contribute to improve detection of imported malaria of the international travelers.
