不同病因行全髋关节置换术患者总失血量和隐性失血量差异分析

摘　　要：目的研究不同病因行全髋关节置换术(THA)患者隐性失血量与总失血量的差异。方法回顾性研究120例行单侧THA治疗的患者的临床资料,根据病因的不同,分为股骨颈骨折(FNF)组78例和髋关节骨性关节炎(RHOA)组42例。比较2组患者隐性失血量、总失血量及隐性失血量在总失血量中的比例。结果120例患者平均隐性失血量为(702.14±98.74)ml,平均总失血量为(1625.06±198.43)ml,隐性失血量在总失血量中的占比为43.21%。FNF组平均隐性失血量为(622.24±102.35)ml,平均总失血量为(1456.76±132.47)ml,隐性失血量在总失血量中的占比为42.71%;RHOA组平均隐性失血量为(749.08±115.65)ml,平均总失血量为(1719.74±184.43)ml,隐性失血量在总失血量中的占比为43.56%。RHOA组隐性失血量和总失血量较FNF组均明显增多(P<0.01),而2组患者手术时间和隐性失血量在总失血量中的比例比较,差异无统计学意义(P>0.05)。结论在行THA治疗的患者中,两种不同病因(FNF与RHOA)的患者隐性失血量和总失血量存在一定差异,其中RHOA患者隐性失血量和总失血量明显多于FNF患者。Objective To investigate the difference between total blood loss and recessive blood loss in patients undergoing total hip arthroplasty(THA)because of different causes.Methods The clinical data about 120 patients who received unilateral THA in our hospital from January 2017 to December 2018 were retrospectively analyzed.According to different causes of disease,the patients were divided into femoral neck fracture group(FNF group,n=78)and radiographic hip osteoarthritis group(RHOA group,n=42).The recessive blood loss volume,the proportion of total blood loss and recessive blood loss in total blood loss were observed and compared between the two groups.Results The average recessive blood loss volume in the 120 patients was(702.14±98.74)ml,the average total blood loss volume was(1,625.06±198.43)ml,and the proportion of recessive blood loss in total blood loss was 43.21%.The average recessive blood loss volume in FNF group was(622.24±102.35)ml,and the average total blood loss was(1,456.76±132.47)ml,and the proportion of recessive blood loss in total blood loss was 42.71%.In contrast with that,the average hidden blood loss in RHOA group was(749.08±115.65)ml,and the average total blood loss was(1,719.74±184.43)ml,accounting for 43.56%of the total blood loss.The recessive blood loss and total blood loss were significantly increased in RHOA group,as compared with those in FNF group(P<0.01),however,there were no significant differences in the operation time and the proportion of recessive blood loss in total blood loss between the two groups(P>0.05).Conclusion In patients who are treated by THA,there are some differences in the recessive blood loss and total blood loss in patients with two different causes of disease(FNF and RHOA),in which,the recessive blood loss and total bleeding loss in patients with RHOA are significantly higher than those in patients with FNF.
