丹参多酚酸盐对冠心病患者造影剂肾损伤的影响

摘　　要：目的探讨丹参多酚酸盐对冠心病患者介入治疗术后造影剂肾损伤的影响。方法选取介入治疗术前和术后静脉滴注丹参多酚酸盐的冠心病患者90例(治疗组),选择同期未应用丹参多酚酸盐的冠心病患者90例为对照(对照组)。术前、术后24 h、术后48 h、术后72 h分别测定2组血肌酐和血清胱抑素C(CysC)。采用MDRD简化公式计算肾小球滤过率(eGFR)。结果治疗组患者术后48 h和术后72 h的血清CysC浓度明显低于对照组(P<0.05)。与术前比较,对照组患者术后24 h和术后48 h的血清CysC浓度均明显升高(P<0.05)。治疗组患者在术后24 h、48 h、72 h时肾功能改善的程度(血肌酐、血清CysC、eGFR改善幅度)明显优于对照组(P<0.05)。结论丹参多酚酸盐对冠心病患者介入治疗后造影剂肾损伤具有保护作用,血清CysC能有效反映造影剂早期肾损伤。Objective To investigate the effects of Salvianolate on contrast-induced renal jury in patients with coronary artery disease(CAD)after percutaneous coronary intervention(PCI).Methods Ninety patients with CAD who received intravenous drip of salvianolate before and after PCI were enrolled as treatment group,at the same time,the other 90 patients with CAD who did not receive Salvianolate medication were enrolled as control group.The serum levels of creatinine and cystatin C(CysC)were measured before PCI,at 24h,48h,72h after PCI.Moreover the glomerular filtration rate(eGFR)was calculated according to MDRD simplified formula.Results The serum levels of CysC in treatment group were significantly lower than those in control group at 48h and 72h after PCI(P<0.05).As compared with those before PCI,the serum levels of CysC in control group were increased significantly at 24h and 48h after PCI(P<0.05).Moreover the improvement degree of renal function(creatinine,serum CysC,eGFR)in treatment group at 24h,48h and 72h after PCI was superior to that in control group(P<0.05).Conclusion Salvianolate has protective effects on contrast-induced renal injury in patients with CAD after PCI,and serum CysC can effectively reflect the early renal injury induced by contrast.
