白三烯受体拮抗剂联合丙酸氟替卡松和依巴斯汀对中重度变应性鼻炎的疗效观察

摘　　要：目的研究白三烯受体拮抗剂孟鲁司特钠、丙酸氟替卡松鼻喷剂及依巴斯汀联用治疗中重度变应性鼻炎的临床疗效。方法将中重度变应性鼻炎患者随机分成试验组及对照组,每组45例。试验组给予孟鲁司特钠及依巴斯汀口服,并联合丙酸氟替卡松鼻喷雾剂喷鼻;对照组给以丙酸氟替卡松鼻喷雾剂外用联合依巴斯汀口服,疗程均为8周。于治疗前、治疗后4周、8周观察2组,鼻部症状视觉模拟量表(VAS)评分、鼻-结膜炎相关生活质量(RQLQ)评分和药物治疗总有效率。结果在治疗4周及8周时,试验组VAS总评分和各项鼻症状VAS评分与对照组比较,差异有统计学意义(P<0.05);治疗8周时2组患者RQLQ评分较治疗前有明显降低,试验组与对照组比较RQLQ评分差异有统计学意义(P<0.05);试验组总有效率显著高于对照组,差异有统计学意义(P<0.05)。结论白三烯受体拮抗剂孟鲁司特钠联合鼻喷激素丙酸氟替卡松及抗组胺剂依巴斯汀在治疗中重度变应性鼻炎时可以协同起效,能够快速减轻AR患者的鼻部不适症状,并对患者生活质量有效改善。Objective To investigate the therapeutic effects of montelukast combined with fluticasone propionate nasal spray and ebastine tablets on moderate and severe allergic rhinitis.Methods A total of 90 patients with moderate and severe allergic rhinitis were randomly divided into experimental group(n=45)and control group(n=45).The patients in experimental group were treated by oral montelukast and ebastine tablets,combining with fluticasone propionate nasal spray,however,the patients in control were treated by fluticasone propionate nasal spray combined with oral ebastin tablets,with a treatment course of 8 weeks for both groups.The visual analogue scale(VAS)scores of nasal symptoms,rhino conjunctivitis related life quality questionnaire(RQLQ)scores and total effective rate were observed and compared between the two groups.Results There were significant differences in the total nasal symptoms VAS scores or single nasal symptoms VAS scores at 4w,8w after treatment between the two groups(P<0.05).After 8-week treatment,the RQLQ scores were significantly decreased in both groups,there were significant differences in RQLQ scores between the two groups(P<0.05).Moreover the total effective rate in experimental group was 95.56%,which was significantly higher than that(80.00%)in control group(P<0.05).Conclusion Leukotriene receptor antagonist-montelukast sodium combined with fluticasone propionate nasal spray and ebastine tablets have synergistic reaction in treatment of moderate and severe allergic rhinitis,which can relieve clinical symptoms quickly and improve the life quality of patients.
