摘　　要：目的探讨改良套圈缝合法治疗儿童手指近侧指间关节侧副韧带损伤临床效果。方法选取2010年12月至2015年2月近指间关节侧副韧带损伤患儿23例(23指),根据手术方式分为对照组(11例11指)和治疗组(12例12指)。对照组采用传统钢丝抽出法,治疗组采用改良套圈缝合法,观察2组治疗疗效。结果患儿术后均得到随访,随访时间9~21个月,平均16.5个月,结果所有随访患者术后切口均Ⅰ期愈合,无感染情况发生,指间关节被动活动稳定、关节ROM 70°~110°,平均ROM为90°;待基本治愈之后,关节处伴随着梭形肿胀的情况,但是关节较为稳健,患者并不会承受疼痛困扰。2组治疗在手术时间,术中出血量及术后Saetta关节功能评分上的比较差异均无统计学意义(P>0.05),但改良套圈缝合法无需再次手术,避免了再次手术的并发症及缩短了愈合时间。结论改良套圈缝合法治疗儿童近指间指间关节侧副韧带损伤的疗效满意,值得推广应用。Objective To investigate the therapeutic effects of modified ring suture technique on proximal interphalangeal collateral ligaments injury in children.Methods A total of 23 children(23 fingers)with proximal interphalangeal collateral ligaments injury who were treated in our hospital from December 2010 to February 2015 were enrolled in the study.According to the operation mode,these patients were divided into two groups.The patients in control group were treated by traditional steel wire pulling method,however,the patients in observation group were treated by improved ring suture method.After treatment the therapeutic effects were observed and compared between the two groups.Results The postoperative follow up was performed for 9~21 months,with an average follow up time being 16.5 months.The results showed that all the patients had postoperative incision stageⅠhealing,without incidence of infaction and pain,after operation the wounded finger joint activity was stable.There were no significant differences in operation time,intraoperative blood loss and postoperative Saetta joint function scores between the two groups(P>0.05).However the modified ring suture technique did not need the reoperation,which could avoid the surgical complications and shorten the healing time.Conclusion The modified ring suture technique is satisfactory in treatment of collateral ligament injury of the proximal interphalangeal joint in children,therefor,it is worthy of promotion and application in clinical practice.
