磁敏感加权成像在脑静脉血管畸形影像学特征分析中的应用

摘　　要：目的:探讨脑静脉血管畸形(CVM)患者应用磁敏感加权成像(SWI)诊断的临床价值及其影像学特征。方法:前瞻性选取在医院就诊的72例高度疑似CVM患者,所有患者在常规检查基础上增加磁共振成像(MRI)平扫及SWI检查,以最终病理诊断结果为"金标准",对比MRI及SWI应用于CVM诊断中的准确率、灵敏度和特异度,分析SWI检查诊断CVM的影像学特征。结果:SWI诊断准确率、灵敏度及特异度与病理诊断比较差异无统计学意义,但准确率明显高于MRI诊断,其差异有统计学意义(t=3.055,P<0.05);病灶分布诊断准确率比较,MRI明显低于SWI和病理诊断,差异有统计学意义(t=3.415,P<0.05)。在72例疑似CVM患者中SWI确诊63例,共检出病灶73个,观察图像发现70个病灶都能清晰显示,呈典型水母头样改变,纤细的深髓静脉即便不注射造影剂均能清晰显示,且病灶显示范围明显大于MRI。结论:使用SWI诊断CVM的准确率、特度度及灵敏度均较高,影像学图像清晰、辨识度高,无需使用对比剂即可显示细小髓静脉,可作为CVM的首选诊断方法。Objective:To investigate the clinical value and imaging characteristics of susceptibility weighted imaging(SWI) in diagnosing cerebral venous malformation(CVM) of patients.Methods:Seventy-two patients who were highly suspected CVM were prospectively selected as the study subjects.All patients were examined by plain scan of MRI and SWI on the basis of routine examinations.The results of final pathological diagnosis were regarded as "gold standard".The accuracies,sensitivities and specificities of MRI and SWI in diagnosis of CVM were compared.And the imaging characteristics of the patients with CVM who underwent examination and diagnosis of SWI were analyzed.Results:There were no significant difference in accuracy,sensitivity and specificity between SWI and pathological diagnosis,but that of SWI was significantly higher than that of MRI diagnosis(t=3.055,P<0.05).The diagnostic accuracy of lesion distribution of MRI was significantly lower than that of SWI and pathological diagnosis(t=3.415,P<0.05),respectively.63 cases were confirmed as CVM by SWI,and a total of 73 lesions were detected,and 70 lesions could be clearly displayed by observing images,and these lesions showed typically jellyfish head-like changes.Fine deep medullary veins could be clearly displayed by SWI even without injection of contrast medium,and the indication scope of lesions of SWI was significantly larger than that of MRI.Conclusion:The accuracy,specificity and sensitivity of SWI in diagnosing CVM are higher.The iconographical images of SWI are clear and highly identifiable.And the fine medullary veins can be displayed by SWI without the use of contrast medium.It can be used as the first choice in diagnosing CVM.
