摘 要:目的:探讨常规M型频谱多普勒超声心动图、彩色M型超声心动图、多普勒组织成像(DTI)等3种超声技术在评估房颤患者左室舒张功能中的应用。方法:选取医院收治的86例房颤患者纳入房颤组,另选同期与房颤组患者资料相匹配的86名健康体检者纳入健康对照组。采用常规M型超声心动图检测两组左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、舒张末期室间隔厚度(IVSTd)、舒张末期左室后壁厚度(LVPWTd),左心房舒张末期面积(LAA)和右心房舒张末期面积(RAA);采用频谱多普勒超声检测二尖瓣血流频谱舒张早期峰值流速(E)、肺静脉血流频谱心室收缩期S波峰值流速(PVS)、舒张期D波峰值流速(PVD),计算收缩期峰值流速与舒张末期流速之比(S/D);采用彩色M型超声心动图检测舒张早期左室内血流传播速度(FPV);采用DTI检测收缩期运动速度(Vs)及舒张期运动速度(Ve),计算舒张期早期血流峰速与舒张期运动速度之比(E/Ve);比较两组各超声技术检测参数间的差异。结果:房颤组与健康对照组比较,LVESD、LVEDD、IVSTd、LVPWTd等参数无明显差异(t=0.522,t=1.637,t=1.435,t=1.769;P>0.05),房颤组LAA和RAA参数高于对照组,差异有统计学意义(t=14.083,t=18.437;P<0.05);两组E值无明显差异(t=1.782,P>0.05),房颤组PVS、S/D低于健康对照组,PVD高于健康对照组,差异均有统计学意义(t=8.150,t=9.293,t=8.028;P<0.05);房颤组FPV、Vs和Ve均低于对照组,E/Ve高于健康对照组,差异均有统计学意义(t=17.581,t=3.317,t=9.333,t=17.892;P<0.05)。结论:房颤患者左心室结构未发生明显改变,彩色M型超声心动图和DTI检测FPV、Vs、Ve及E/Ve参数用于评估房颤患者左室舒张功能具有一定优势。Objective:To investigate the applications of three kinds of ultrasound techniques included conventional M-type spectral Doppler echocardiography,color M-type echocardiography and Doppler tissue imaging(DTI)in the assessment of left ventricular diastolic function of patients with atrial fibrillation.Methods:86 patients with atrial fibrillation in hospital were included in atrial fibrillation group.And 86 healthy people who underwent physical examinations and whose data were matched with those of atrial fibrillation group were included in healthy control group.The left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD),interventricular septum thickness at end-diastole(IVSTd),left ventricular posterior wall thickness at end-diastole(LVPWTd),left atria end-diastolic area(LAA)and right atria end-diastolic area(RAA)of two groups were detected by conventional M-type echocardiography.The mitral blood flow spectral early peak flow velocity(E),pulmonary vein systolic S wave velocity(PVS)and pulmonary vein diastolic D wave velocity(PVD)were detected by spectral Doppler ultrasonography.The ratio of peak systolic velocity to peak end diastolic velocity(S/D)was calculated.Color M-type echocardiography was used to detect early diastolic left ventricular flow propagation velocity(FPV).DTI was performed to detect peak velocities of systolic term(Vs)and end-diastolic velocity(Ve).The ratio of peak velocity of diastolic early term to diastolic motion velocity(E/Ve)was calculated.The differences of detection parameters of each ultrasound technique between two groups were compared.Results:The differences of LVESD,LVEDD,IVSTd and LVPWTd between the two groups were no significant(t=0.522,t=1.637,t=1.435,t=1.769,P>0.05).LAA and RAA of atrial fibrillation group were significantly higher than those of control group(t=14.083,t=18.437,P<0.05),respectively.There was no significant difference in E value between the two groups(t=1.782,P>0.05).The PVS and S/D of atrial fibrillation group were significantly l
