摘 要:目的研究宫颈分泌物沙眼衣原体(CT)、解脲脲原体(UU)及人型支原体(Mh)检测结果与不孕症间的相关性。方法将2017年1月-2018年1月收治的316例女性不孕症患者纳为不孕组,同期于妇科门诊要求终止妊娠的孕早期146例患者纳为早孕组,94例输卵管性不孕患者纳为输卵管性不孕组,103例腹腔镜手术者纳为手术组。比较不孕组与早孕组宫颈分泌物CT、UU及Mh培养阳性率;比较输卵管不孕组与手术组宫颈分泌物及盆腔积液CT、UU及Mh培养阳性率。根据患者输卵管功能将输卵管性不孕者分为低分组(24例)、中分组(26例)与高分组(44例),观察3组宫颈分泌物及盆腔积液CT、UU及Mh培养阳性率。结果不孕组CT培养阳性率为12.97%,支原体感染率为41.14%,均显著高于早孕组(均P<0.05);输卵管性不孕组宫颈分泌物CT阳性率为11.70%,支原体感染率为36.17%,盆腔积液CT感染率为5.32%,均显著高于手术组(均P<0.05);低分组、中分组及高分组输卵管性不孕患者宫颈分泌物CT培养阳性率及支原体感染率依次升高,组间差异均有统计学意义(均P<0.05)。结论女性不孕症及输卵管性不孕患者宫颈分泌物CT、UU及Mh感染率均明显高于正常水平,且患者输卵管功能越差,宫颈分泌物CT、UU及Mh感染率越高,提示CT、UU及Mh感染与不孕间关系密切,可能是诱发并加重输卵管障碍的原因之一。Objective To research the correlation between detection of cervical secretions of Chlamydia trachomatis(CT),Ureaplasma urealyticum(UU),Mycoplasma hominis(Mh)and infertility.Methods From January 2017 to January 2018,316 patients with female infertility were selected as infertility group,146 patients terminating the pregnancy in early pregnancy in Outpatient of Gynecology were selected as early pregnancy group,94 patients with tubal infertility were selected as tubal infertility group,and 103 patients undergoing laparoscopic surgery were selected as surgery group.The culture positive rates of CT,UU,and Mh in cervical secretions of patients in infertility group and early pregnancy group were compared.The culture positive rates of CT,UU,and Mh in cervical secretions and pelvic fluid of patients in tubal infertility group and surgery group were compared.According to tubal function,the patients in tubal infertility group were divided into low-score group(24 patients),middle-score group(26 patients),and high-score group(44 patients),the culture positive rates of CT,UU,and Mh in cervical secretions and pelvic fluid of patients in the three groups were observed.Results The culture positive rate of CT and infection rate of mycoplasma in infertility group were 12.97%and 41.14%,respectively,which were statistically significantly higher than those in early pregnancy group(P<0.05).The positive rate of CT in cervical secretions,the infection rate of mycoplasma,the infection rate of CT in pelvic fluid of patients in tubal infertility group were 11.70%,36.17%,and 5.32%,respectively,which were statistically significantly higher than those in surgery group(P<0.05).The culture positive rates of CT in cervical secretions and infection rates of mycoplasma in low-score group,middle-score group,and high-score group increased in turn,there were statistically significant differences(P<0.05).Conclusion The infection rates of CT,UU,and Mh in cervical secretions of patients with female infertility and tubal infertility are significantly high
