摘　　要：目的探讨膝关节镜下关节清理术联合腓骨近端截骨治疗膝关节骨性关节炎的临床效果。方法采用便利抽样法,选取我院2016年1月~2019年1月收治的80例膝关节骨性关节炎患者作为研究对象,依据随机数字表法分为观察组与对照组,各40例。对照组患者采用膝关节镜下关节清理术治疗,观察组采用膝关节镜下关节清理术联合腓骨近端截骨治疗。两组患者均随访6个月,比较两组患者的治疗效果、治疗前后疼痛评分及并发症总发生率。结果观察组患者的总有效率为90.0%,高于对照组的67.5%,差异有统计学意义(P<0.05)。两组患者治疗前的视觉模拟量表(VAS)评分比较,差异无统计学意义(P>0.05)。两组患者治疗后6个月的VAS评分低于治疗前,差异有统计学意义(P<0.05)。观察组治疗后6个月的VAS评分低于对照组,差异有统计学意义(P<0.05)。两组的并发症总发生率比较,差异无统计学意义(P>0.05)。结论膝关节镜下关节清理术联合腓骨近端截骨治疗膝关节骨性关节炎的临床效果显著,能够缓减患者疼痛,且不增加并发症,值得临床推广。Objective To explore the clinical effect of arthroscopic knee joint debridement combined with proximal fibular osteotomy in the treatment of knee osteoarthritis.Methods Using convenience sampling method,80 patients with knee osteoarthritis admitted to our hospital from January 2016 to January 2019 were selected as the research objects.According to random number table method,they were divided into the observation group and the control group,40 cases in each group.The control group was treated with arthroscopic debridement of knee joint.The observation group was treated by arthroscopic knee joint debridement combined with proximal fibular osteotomy.The two groups of patients were followed up for 6 months.The therapeutic effect,pain score before and after treatment and the total incidence of complications were compared between the two groups.Results The total effective rate of the observation group was 90.0%,which was higher than 67.5%of the control group,the difference was statistically significant(P<0.05).There was no significant difference in visual analogue scale(VAS)score between the two groups before treatment(P>0.05).The VAS scores of the two groups 6 months after treatment were lower than those before treatment,the differences were statistically significant(P<0.05).The VAS score of the observation group 6 months after treatment was lower than that of the control group,the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion The clinical effect of arthroscopic knee joint debridement combined with proximal fibular osteotomy in the treatment of knee osteoarthritis is significant,which can relieve pain without increasing complications,it is worthy of clinical promotion.
