胎膜早破孕妇排尿困难的高危影响因素分析

摘　　要：目的分析胎膜早破孕妇排尿困难的高危影响因素,并根据这些影响因素提出相应的护理对策。方法回顾性分析2017年10月~2018年2月我院收治135例胎膜早破孕妇临床资料进行分析,评估患者的排尿情况,对排尿困难的影响因素进行分析。结果所有患者中有34例发生排尿困难,发生率为25.19%(34/135)。单因素分析,孕妇的排尿困难的高危影响因素包括分娩经历、排尿姿势、破膜后首次排尿时间、卧床待产时间,与未发生排尿困难的孕妇比较,差异有统计学意义(P<0.05)。多因素分析,孕妇分娩经历初产、卧床待产时间≥5 h、破膜后首次排尿时间≥3 h、排尿姿势仰卧位是孕妇排尿困难的影响因素(P<0.05)。结论胎膜早破孕妇发生排尿困难的高危因素包括分娩经历初产、卧床待产时间≥5 h、破膜后首次排尿时间≥3 h、排尿姿势仰卧位等。有效调整排尿姿势,加强孕妇的心理疏导和健康宣教工作,督促孕产妇及时排尿,可使排尿困难显著减少。Objective To analyze the high risk factors of dysuria in pregnant women with premature rupture of membranes,and put forward corresponding nursing countermeasures according to these high risk factors.Methods The clinical data of 135 pregnant women with premature rupture of membranes treated in our hospital from October 2017 to February 2018 were retrospectively analyzed.The urination status of the patients was evaluated and the influencing factors of dysuria were analyzed.Results Among the participants,34 pregnant women had dysuria,the incidence was 25.19%(34/135).Under univariate analysis,the high risk factors of dysuria in pregnant women included childbirth experience,posture of urination,first urination time after rupture of membranes,and waiting time in bed.There was significant difference between pregnant women with dysuria and those without dysuria(P<0.05).The first delivery experience,waiting time in bed≥5 h,first uriration time after of member≥3 h,and urination posture of urination were high factors for dysuria in pregrant women(P<0.05).Conclusion The high risk factors of dysuria in pregnant women with premature rupture of membranes include the first delivery experience,waiting time in bed≥5 h,first urination time after rupture of membranes≥3 h,posture of urination of urination,etc.Effective adjustment of urination posture,strengthening psychological counseling and health education for pregnant and lying-in women,and urging pregnant and lying-in women to urinate in time can significantly reduce the difficulty of urination.
