摘　　要：目的对麻疹疑似病例中发热伴出疹疾病进行相关病毒检测,探讨在麻疹消除阶段麻疹疑似病例的病原构成.方法收集北京市2017-2018年406份麻疹疑似病例咽拭子,用实时荧光PCR法进行麻疹病毒、风疹病毒、水痘-带状疱疹病毒、人细小病毒B19、EB病毒、其他肠道病毒等11种病毒核酸检测.结果除肠道病毒71型外的10种病毒均检测到,共145例阳性,总阳性率35.71%.除麻疹病毒和风疹病毒外,检出前3位的是单疱病毒6型(13例,3.20%)、EB病毒(12例,2.96%)和人细小病毒B19(10例,2.46%).其他肠道病毒共21例,阳性率5.17%.麻疹病毒、风疹病毒、水痘-带状疱疹病毒、人细小病毒B19和其他肠道病毒发病以成人为主,EB病毒、单疱病毒6型、柯萨奇A组16型儿童和成人病例比例相似.结论除肠道病毒71型外的10种病毒能引起类似麻疹症状而被临床诊断为麻疹.其他肠道病毒也能引起类似麻疹的全身出疹性疾病,而且比例较高.Objective To detect rash and fever illness(RFIs)associated viruses in suspected measles cases,and to explore the pathogenic spectrum among suspected measles cases in the elimination stage of measles.Methods A total of 406 pharyngeal swab samples of suspected measles cases in Beijing from 2017 to 2018 were collected.The nucleic acids of 11 RFIs associated viruses,including measles virus,rubella virus,varicella-zoster virus,human parvovirus B19,EB virus and other enteroviruses were detected by real-time PCR.Results Ten viruses,except for enterovirus 71,were detected.One hundred and forty-five cases were positive,with an overall positive rate of 35.71%.The top three viruses after measles virus and rubella virus were 13 cases of herpes simplex virus 6(3.20%),12 cases of EB virus(2.96%)and 10 cases of human parvovirus B19(2.46%).Tweenty-one cases were positive for other enteroviruses and the positive rate was 5.17%.The cases infected measles virus,rubella virus,varicella-zoster virus,human parvovirus B19 and other enteroviruses were mainly adults,while the proportions of EB virus,herpes simplex virus 6 and coxsackievirus A6 infections in children and in adults were similar.Conclusion Infections by 10 viruses,except for enterovirus 71 may cause symptoms similar to measles and may be clinically diagnosed as measles.Other enteroviruses can also cause measles-like systemic rash diseases,and the proportion was high.
