摘  要:目的观察2型糖尿病并发冠心病患者血浆miRNA-let7(miR-let7)家族表达变化,分析miR-let7对2型糖尿病并发冠心病的诊断价值。方法选取2型糖尿病并发冠心病患者60例纳入T2DM+CHD组,2型糖尿病患者76例纳入T2DM组,另选健康体检者88例为对照组。采用荧光定量PCR法检测各组血浆miR-let7家族。采用受试者工作特征(ROC)曲线分析血浆miR-let7对糖尿病并发冠心病的诊断价值。结果T2DM+CHD组患者血浆miR-let7b、miR-let7i相对表达量高于T2DM组,miR-let7g、miR-let7f-1相对表达量低于T2DM组(P均<0.01)。miR-let7b、miR-let7f-1、miR-let7g、miR-let7i诊断T2DM的ROC曲线下面积(AUC)分别为0.9378(95%CI 0.8466~1.0290,P<0.01)、0.9976(95%CI 0.9898~1.0050,P<0.01)、0.8397(95%CI 0.7063~0.9731,P<0.01)、0.9254(95%CI 0.8504~1.0200,P<0.01);miR-let7b、miR-let7f-1、miR-let7g、miR-let7i诊断T2DM+CHD的AUC分别为0.8091(95%CI 0.8466~1.0290,P<0.01)、0.8727(95%CI 0.7322~1.0130,P<0.01)、0.7515(95%CI 0.5588~0.9442,P<0.01)、0.8121(95%CI 0.6452~0.9791,P<0.01)。结论与单纯2型糖尿病患者相比,2型糖尿病并发冠心病患者血浆miR-let7b、miR-let7i表达升高,miR-let7g、miR-let7f-1表达降低;检测血浆miR-let7b、miR-let7f-1、miR-let7g、miR-let7i有助于2型糖尿病并发冠心病的诊断。Objective To observe the changes of plasma miRNA-let7(miR-let7)family expression in patients with type 2 diabetes mellitus(T2DM)complicated with coronary heart disease(CHD),and to analyze the diagnostic value of miR-let7 in T2DM complicated with CHD.Methods Sixty patients with T2DM complicated with CHD in the T2DM+CHD group,76 patients with T2DM in the T2DM group,and 88 healthy people in the control group were selected.The plasma miR-let7 family was detected by real-time PCR.The value of plasma miR-let7 in diagnosis of T2DM complicated with CHD was analyzed using receiver operating characteristic(ROC)curve.Results The relative expression of miR-let7b and miR-let7i in the T2DM+CHD group was higher than that in T2DM group,and the relative expression of miR-let7g and miR-let7f-1 was lower than that in T2DM group(all P<0.01).The area under the ROC curve(AUC)for the diagnosis of T2DM by miR-let7b,miR-let7f-1,miR-let7g,and miR-let7i were 0.9378(95%CI:0.8466-1.0290,P<0.01),0.9976(95%CI:0.9898-1.0050,P<0.01),0.8397(95%CI:0.7063-0.9731,P<0.01),and 0.9254(95%CI:0.8504-1.0200,P<0.01);the AUC for the diagnosis of T2DM+CHD by miR-let7b,miR-let7f-1,miR-let7g,and miR-let7i were 0.8091(95%CI 0.8466-1.0290,P<0.01),0.8727(95%CI 0.7322-1.0130,P<0.01),0.7515(95%CI 0.5588-0.9442,P<0.01),and 0.8121(95%CI 0.6452-0.9791,P<0.01).Conclusion Compared with patients with T2DM,the expression of miR-let7b and miR-let7i increases,and the expression of miR-let7g and miR-let7f-1 decreases in patients with T2DM complicated with CHD;detecting the plasma miR-let7b,miR-let7f-1,miR-let7g,and miR-let7i contributes to the diagnosis of T2DM complicated with CHD.
