摘  要:目的观察益气化瘀中药对糖尿病溃疡大鼠瘢痕形成的影响,并探讨其机制。方法将50只大鼠随机分为益气化瘀组、益气组、化瘀组、模型组、正常对照组,每组10只。益气化瘀组、益气组、化瘀组、糖尿病模型组通过腹腔注射链脲佐菌素结合外科手术建立糖尿病背部皮肤溃疡大鼠模型。正常对照组、糖尿病模型组以生理盐水灌胃,1次/d;益气化瘀组、益气组、化瘀组分别给予益气化瘀方2.5 g/mL、益气方1.5 g/mL、化瘀方1.0 g/mL灌胃,1次/d。在创面愈合1个月时处死大鼠,剪取背部全层皮肤瘢痕组织,测量瘢痕厚度并计算瘢痕指数;取瘢痕组织进行HE染色,检测Ⅰ型胶原含量;采用Western blotting法检测瘢痕组织中的激活素受体样激酶5(ALK5)、Smad2、Smad3蛋白。结果益气化瘀组、益气组、化瘀组及正常对照组的瘢痕厚度、瘢痕指数均低于糖尿病模型组(P均<0.01)。益气化瘀组、益气组、化瘀组瘢痕组织中Ⅰ型胶原含量低于糖尿病模型组(P均<0.05)。益气化瘀组、益气组、化瘀组瘢痕组织中ALK5、Smad3相对表达量低于糖尿病模型组(P均<0.05),益气化瘀组瘢痕组织中Smad2相对表达量低于糖尿病模型组(P<0.05)。结论益气化瘀中药有助于延缓糖尿病溃疡大鼠瘢痕形成,机制可能与干预TGF-β/ALK5/Smad2/3通路、调节Ⅰ型胶原含量有关。Objective To explore the effect of Chinese herbs for replenishing qi and resolving stagnation on scar formation in diabetic ulcer rats and to explore its mechanism.Methods Fifty rats were randomly divided into five groups:replenishing qi and resolving stagnation treatment group,replenishing qi treatment group,resolving stagnation treatment group,model group,and normal control group.We established the diabetic ulcer rat models by intraperitoneal injection of streptozotocin combined with surgery in the former four groups.Then the rats in the normal control group and model group were given normal saline by gavage for 1 time/d.Meanwhile,the rats in the replenishing qi and resolving stagnation treatment group,replenishing qi treatment group,and resolving stagnation treatment group were given 2.5 g/mL,1.5 g/mL,and 1.0 g/mL TCM herbs differently by gavage for 1 time/d.The rats were executed when the wound healed for 1 month.The whole layer of skin scar tissue was cut off from the back.The scar thickness was measured and the scar index was calculated,and the expression of activin receptor-like kinase(ALK)5,Smad2,and Smad3 protein was detected by Western blotting.Results The scar thickness and scar index of the replenishing qi and resolving stagnation treatment group,replenishing qi treatment group,resolving stagnation treatment group,and normal control group were lower than that of the model group(both P<0.01).The levels of collagen type I,ALK5,and Smad3 of the three TCM herb groups were lower than those of the model group(all P<0.05).The relative expression of Smad2 in the replenishing qi and resolving stagnation treatment group was lower than that in the model group(P<0.05).Conclusion Replenishing qi and resolving stagnation therapy perhaps can delay scar formation in diabetic ulcer rats by interfering TGF-β/ALK5/Smad2/3 and regulating collagen type I.
