四神丸汤联合耳穴压豆治疗ICU患者脾肾阳虚型腹泻临床研究

摘　　要：目的:观察四神丸汤联合耳穴压豆治疗重症加强护理病房(ICU)患者脾肾阳虚型腹泻的临床疗效。方法:将118例患者随机分为研究组和对照组,各59例,均积极治疗原发病,对照组予以蒙脱石散治疗,研究组在对照组的基础上加用四神丸汤联合耳穴压豆治疗,治疗2周后观察比较2组临床疗效、治疗前后主症积分、IBS大便性状问卷(IBS-SSS)评分、血沉(ERS)、C-反应蛋白(CRP)的变化情况。结果:总有效率研究组为89.83%,对照组为62.71%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组患者大便溏薄、泄泻不止、腹部冷痛、形寒肢冷等主症积分均较治疗前明显降低(P<0.05),且研究组各项症状评分均低于对照组(P<0.05)。治疗后,2组患者IBS-SSS评分、ERS及CRP水平均较治疗前明显降低(P<0.05),且研究组各项指标均低于对照组(P<0.05)。结论:四神丸汤联合耳穴压豆治疗ICU患者脾肾阳虚型腹泻疗效显著,可显著改善患者临床症状,恢复大便性状。Objective:To observe the clinical effect of the therapy of Sishenwan Tang combined with auricular point pressing with beans for diarrhea of yang deficiency of spleen and kidney type in ICU patients.Methods:A total of 118 cases of patients who actively received treatment for the primary disease were randomly divided into the study group and the control group,59 cases in each group.The control group was treated with Montmorillonite powder and the study group was additionally treated with Sishenwan Tang combined with auricular point pressing with beans based on the treatment of the control group.After two-week treatment,the clinical effect and the changes in the score of main symptoms,the score of Irritable Bowel Syndrome-Severity Scoring System(IBS-SSS),the level of erythrocyte sedimentation rate(ERS) and the level of C-reactive protein(CRP) before and after treatment were observed and compared.Results:The total effective rate was89.83% in the study group and 62.71% in the control group,the difference being significant(P<0.05).After treatment,the scores of main symptoms including loose stool,diarrhea,abdominal cold and pain,and cold body and extremities were significantly decreased when compared with those before treatment(P<0.05),and the symptom scores in the study group were lower than those in the control group(P<0.05).After treatment,the IBS-SSS scores,the ERS levels and the CRP levels in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the above indexes in the study group were lower than those in the control group(P<0.05).Conclusion:The therapy of Sishenwan Tang combined with auricular point pressing with beans has significant therapeutic effect in treating ICU patients with diarrhea of yang deficiency of spleen and kidney type,which can significantly improve their clinical symptoms and restore the property of stool.
