米力农短期治疗扩张型心肌病心力衰竭的临床疗效与安全性

摘　　要：目的观察米力农短期治疗扩张型心肌病心力衰竭患者的临床疗效,并对安全性做出评价。方法研究对象为我院2018年9月至2019年5月收治的72例扩张型心肌病心力衰竭患者,采用随机数字表法分为对照组和米力农组,每组36例。对照组给予ACEI或ARB、地高辛、利尿剂等常规治疗,米力农组在此基础上给予米力农治疗。比较两组治疗后临床疗效、左室舒张末内径(LVDd)、左室射血分数(LVEF),检测血清氨基末端B型利钠肽前体(NT-proBNP),并观察安全性。结果治疗后,对照组总有效率为63.89%,米力农组总有效率为86.11%,两组比较差异有统计学意义(P<0.05)。米力农组LVDd与对照组比较未见缩小(P>0.05),而LVEF较对照组增加(P<0.05)。血清NT-proBNP水平较基线水平降低(P<0.05),米力农组NT-proBNP较对照组降低(P<0.05)。两组无明显不良反应。结论米力农短期应用可明显改善扩张型心肌病心力衰竭患者的临床症状,改善心脏收缩功能,未见明显不良反应。Objective To explore the clinical efficacy and safety of milrinone in the short-term treatment of dilated cardiomyopathy(DCM)heart failure,and to evaluate the safety.Methods A total of 72 patients suffering from DCM with heart failure who were admitted to our hospital from September 2018 to May 2019 were chosen.The patients were randomly divided into control group and milrinone group with 36 cases in each group.Control group was treated with ACEI or ARB digoxin and diuretic.Milrinone group was treated with milrinone plus the drugs for control group.The clinical efficacy,left ventricular end-diastolic diameter(LVDd),left ventricular ejection fraction(LVEF)and N-terminal pronatriuretic peptide(NT-proBNP)were compared before and after treatment.The safety of milrinone was observed too.Results The total effective rates of control group and milrinonet group were 63.89%and 86.11%,respectively,there was a marked difference between the two groups(P<0.05).Compared with control group,the LVEF was significantly improved(P<0.05)and the NT-proBNP was obviously reduced(P<0.05)after treatment in milrinone group,but the LVDd was not reduced.No obvious adverse reaction was observed during medication.Conclusion The short-term use of milrinone can significantly improve the clinical symptoms of patients with DCM heart failure and improve the systolic function of heart without obvious adverse reactions.
