首次冠状动脉CTA检查失败的相关因素分析与干预对策

摘　　要：目的对首次冠状动脉CTA检查失败的相关影响因素进行分析,并给予相应护理干预。方法选取我院2016年1月至2018年12月接收的首次行冠状动脉CTA检查患者180例为研究对象,将检查成功140例设为对照组,检查失败40例设为研究组,对两组临床资料进行logistic回归分析,获取首次行冠状动脉CTA检查失败相关因素。结果两组患者年龄、病程、性别、临床表现比较差异无统计学意义(P> 0. 05);而研究组血压明显高于对照组,呼吸、心率以及造影剂推注速率快于对照组,差异有统计学意义(P <0. 05);心率、呼吸、血压以及造影剂推注速率为首次行冠状动脉CTA检查失败的相关因素;经logistic回归分析,呼吸、心率为影响冠状动脉CTA检查失败的独立危险因素(P <0. 05)。结论首次冠状动脉CTA检查失败的独立危险因素为心率、呼吸,给予呼吸训练、心率控制有利于检查顺利完成。Objective To analyze the related factors affecting the failure of the first coronary CTA examination and give a corresponding nursing intervention. Methods We selected 180 cases of patients with coronary artery CTA who were admitted to our hospital from January 2016 to December 2018 as a study object. After examining successfully,140 cases of patients were selected as the control group,and 40 cases of patients with failure of the examination were included in the study group. The clinical data were analyzed by logistic regression analysis to obtain the first factors associated with the failure of coronary CTA examination. Results There were no significant differences in age,disease duration,gender,and clinical manifestations between the two groups( P > 0. 05). The blood pressure in the study group was obviously higher than that in the control group,and the respiration,heart rate and propellant speed of contrast media in the study group were faster than those in the control group. The differences were statistically significant( P < 0. 05). The heart rate,respiration,blood pressure,and propellant speed of contrast media were the factors associated with the failure of the first coronary CTA. Logistic regression analysis showed that the respiration and heart rate were the independent risk factors associated with the failure of the coronary CTA( P < 0. 05).Conclusion The independent risk factors for the failure of the first coronary CTA examination were heart rate,respiration,supply of breathing training,and heart rate control.
