早期小切口大隐静脉高位结扎剥脱术治疗伴急性血栓性浅静脉炎大隐静脉曲张的临床疗效

摘　　要：目的观察早期小切口大隐静脉高位结扎剥脱术治疗伴急性血栓性浅静脉炎大隐静脉曲张的临床疗效。方法随机选取本院2017年4月至2018年4月接受的84例伴急性血栓性浅静脉炎的大隐静脉曲张患者作为本次研究对象,按照术式不同将所有患者分为对照组(42例,传统治疗)与实验组(42例,早期小切口大隐静脉高位结扎剥脱术治疗),比较两组患者预后情况。结果实验组患者平均术中出血量、抗菌素使用时间、首次下床活动时间、住院时间均明显少于对照组,差异具有统计学意义(P<0.05)。实验组患者治疗后并发症发生率(7.14%)明显低于对照组(16.67%),差异具有统计学意义(P<0.05)。实验组患者术后治愈率(97.62%)高于对照组(83.33%),差异具有统计学意义(P<0.05)。实验组患者术后2周、4周VCSS评分明显优于对照组,差异具有统计学意义(P<0.05)。结论对于伴急性血栓性浅静脉炎的大隐静脉曲张患者行早期小切口大隐静脉高位结扎剥脱术治疗疗效明显优于传统手术,效果显著,且并发症较少。Objective To observe the effect of early small incision high ligation of great saphenous vein stripping great saphenous varicose vein surgery and clinical curative effect with acute superficial thrombophlebitis. Methods From April 2017 to April 2018, 84 patients with acute superficial thrombophlebitis and varicose great saphenous vein were studied. All patients were divided into control group and experimental group according to different surgical methods. 42 patients in control group were given traditional treatment. 42 patients in the experimental group were treated with early small incision high ligation and stripping of great saphenous vein. The prognosis of the two groups was compared. Results The average intraoperative bleeding volume, antibiotic usage time, first time out of bed activity time and hospitalization time of the experimental group were significantly less than those of the control group, the difference was statistically significant(P<0.05). The incidence of complications after treatment in the experimental group(7.14%) was significantly lower than that in the control group(16.67%), the difference was statistically significant(P<0.05). The cure rate of the experimental group(97.62%) was higher than that of the control group(83.33%). The difference was statisti-cally significant(P<0.05).The VCSS score of the experimental group was significantly better than that of the control group at 2 and 4 weeks after operation(P<0.05).Conclusion The effect of early small incision high ligation and stripping of great saphenous vein in patients with acute superficial thrombophlebitis is better than that of traditional operation,with significant efficacy and less complications.
