摘  要:目的:测定原发性三叉神经痛(primary trigeminal neuralgia, PTN)病人血液和脑脊液中降钙素基因相关肽(calcitonin gene-related peptide, CGRP)和P物质(substance P, SP)的含量,探讨CGRP和SP在PTN发作过程中的作用。方法:诊断为PTN的住院病人共50例。术前记录疼痛视觉模拟评分(visual analogue scale, VAS),并抽取同侧颈外静脉血;行三叉神经节射频热凝术,术中穿刺如果有脑脊液,抽取0.5 ml;疼痛缓解后抽取颈外静脉血。结果:术前颈外静脉血CGRP、SP含量高于术后(P <0.01)。术中脑脊液CGRP、SP含量与术前颈外静脉内含量正相关(P<0.01)。结论:CGRP与SP可能参与了三叉神经痛疼痛发作过程。Objective: To measure calcitonin gene-related peptide(CGRP) and substance p(SP) in the blood and cerebrospinal uid of patients and to investigate the roles of CGRP and SP in the development of primary trigeminal neuralgia. Methods: A total of 50 patients with primary trigeminal neuralgia who underwent 3 D CT-guided percutaneous radiofrequency thermocoagulation(PRT) of trigeminal ganglion were enrolled in our study. Before the operation, ipsilateral external jugular vein blood was collected and visual analogue scale(VAS) score was assessed. All patients underwent PRT by a single surgeon. Cerebral spinal fluid(CSF) was taken during the operation. After the operation, the external jugular venous blood was collected and VAS score was assessed, again. Results: The preoperative concentration of CGRP and SP in external jugular serum was higher than that of post-operation(P < 0.01). There was a positive correlation between the concentration of CGRP and SP in external jugular venous serum before operation and that in CSF during surgical procedures(P < 0.01). Conclusions: CGRP and SP may be involved in the development of trigeminal neuralgia.
