血压晨峰对高血压合并冠心病的影响

摘　　要：目的观察24h动态血压监测(ABPM)在高血压合并冠心病患者的血压晨峰特点。方法选取2014年6月~2016年5月我院收治的冠状动脉造影检查高血压患者226例作为研究对象,根据疾病类型将其分为W组(高血压伴冠心病)110例与M组(单纯高血压)116例,进行24 h动态血压监测。结果经过对比显示,W组患者24 h动态血压、昼夜、夜间等不同时段的平均收缩压和舒张压均显著高于M组,差异有统计学意义(P<0.05)。且W组收缩压和舒张压的非杓型比例、晨峰出现频率要远高于M组,差异有统计学意义(P<0.05)。结论通过本实验的研究,可以对高血压合并冠心病患者的血压晨峰以及血压昼夜的变化情况进行分析,明确晨峰对高血压合并冠心病造成的影响,调整对疾病的治疗方案,降低心血管事件的发生。Objective To observe the morning blood pressure peak characteristics of 24-hour ambulatory blood pressure monitoring(ABPM)in patients with hypertension and coronary heart disease.Methods The research group set 110 patients with hypertension and coronary heart disease as group W according to the course of disease for 226 patients.116 patients with simple hypertension were enrolled in group M for 24h ambulatory blood pressure monitoring.Results After comparison,the average systolic and diastolic blood pressure of patients in group w at different periods such as 24h ambulatory blood pressure,day and night were significantly higher than those in group m(P<0.05),with statistical significance.In addition,the non-dipper proportion of systolic and diastolic blood pressure and the frequency of morning breeze in group w are much higher than those in group m(P<0.05),which is statistically significant.Conclusion Through the research of this experiment,the morning breeze of blood pressure and the changes of blood pressure day and night in patients with hypertension complicated with coronary heart disease can be analyzed,the influence of morning breeze on hypertension complicated with coronary heart disease can be determined,the treatment scheme for diseases can be adjusted,and the occurrence of cardiovascular events can be reduced.
