改良式负压封闭持续冲洗引流在慢性伤口的应用及护理

摘　　要：目的探讨改良式负压封闭持续冲洗引流在慢性伤口的应用及护理方法。方法回顾2016年1月至2018年10月我科应用改良式负压封闭持续冲洗引流技术治疗慢性伤口30例,均在彻底清创后行改良式负压封闭持续冲洗引流术治疗1-3周。结果 30例拆除装置后,创面均新鲜肉芽组织生长良好。15例患者创面成功修复;5例患者行游离植皮术;10例患者行皮瓣移植术,术后均伤口愈合良好。结论改良式负压封闭持续冲洗引流不仅保持引流通畅,不易堵塞管腔的优点,而且能及时清除伤口坏死组织和渗出物。伤口的湿性环境加速创面愈合,也为进一步手术治疗做好创面床准备,加快创面愈合速度。改良式负压封闭持续冲洗引流操作方便,所用材料价格低廉,降低患者住院费用,减少患者换药次数,减轻患者痛苦,疗效好,值得推广。有效安全的是取得疗效的关键。Objective To explore application and nursing methods of improved vacuum sealing continuous irrigation and drainage in chronic wounds. Methods Review 30 cases chronic wounds patients treated with modified vacuum sealing continuous irrigation and drainage in our department from January 2016 to October 2018, who were treated with modified vacuum sealing continuous irrigation and drainage for 1-3 weeks after complete debridement. Results Fresh granulation tissue grew well of all 30 cases after device removal. 15 cases were recovered successfully, 5 cases were treated with free skin grafting and 10 cases with skin flap transplantation. All wounds healed well after operation. Conclusion Improved vacuum sealing continuous irrigation and drainage not only has advantages of keeping smooth drainage and no blocking lumen, but can remove necrotic tissue and exudates of wound in time. Wet environment of wound accelerates healing, and prepares wound bed for further surgical treatment to speed up wound healing. Improved negative pressure sealing continuous irrigation and drainage is easy to operate, can reduce hospitalization cost and dressing changes with cheap materials, which can alleviate pains of patients with good curative effect, and is worthy of promotion. Effect and safe treatment is the key to achieve effective treatment.
