摘　　要：目的探讨中性粒细胞/淋巴细胞比值(NLR)与类风湿性关节炎(RA)的相关性。方法选取RA患者220例,其中活动组107例和非活动组113例,同时选取110例健康者为对照组,收集空腹血样本用于血常规、超敏CRP、补体C3(C3)、补体C4(C4)、类风湿因子(RF)、血沉(ESR)、抗CCP抗体等检测,根据血常规结果计算NLR。结果 RA活动组NC、NLR、超敏CRP、ESR、C3、C4、抗CCP抗体和RF均高于RA非活动组和对照组,而LC低于RA非活动组和对照组(P<0.05);在RA活动组患者中,NLR与超敏CRP、ESR、C3、C4、抗CCP抗体、RF和DAS28评分均呈正相关(P<0.05);Logistic回归分析提示校正超敏CRP、抗CCP抗体、DAS28评分等影响因素后, NLR仍然是RA病情活动的独立危险因素(OR=2.753,P<0.01);ROC曲线分析发现,NLR的曲线下面积为0.785(95%CI:0.716~0.854,P<0.01),但其最佳临界值为3.04时,NLR评估RA病情活动性的灵敏度为81.40%,特异度为68.20%。结论 NLR可作为评估RA病情活动的一种有效的炎症标记物。Objective To explore the relationship between neutrophil-to-lymphocyte ratio(NLR)and rheumatoid arthritis(RA).Methods A total of 220 patients with RA were divided into patients with RA activity(n=107)and RA non activity(n=113).110 healthy people were also enrolled in our study.Fasting blood samples were collected for the detection of blood routine,hypersensitive CRP,complement C3(C3),complement C4(C4),rheumatoid factor(RF),erythrocyte sedimenta?tion rate(ESR),anti-CCP antibody,and NLR was calculated according to blood routine results.Results The NC,NLR,high-sensitivity CRP,ESR,C3,C4,anti-CCP antibody and RF in the RA group were higher than those in the RA non-active group and the control group,while LC was lower than the RA inactive group and the control group(P<0.05).NLR was positively correlated with high-sensitivity CRP,ESR,C3,C4,anti-CCP,RF and DAS28 scores in the RA active group(P<0.05).Logistic regression analysis showed that after adjusting hs-CRP,anti-CCP antibodies,DAS28 score,NLR was an independent risk factor of the disease activity of RA(OR=2.753,P<0.01).ROC curve analysis showed that the area under the curve of NLR was 0.785(95%CI:0.716-0.854,P<0.01),but the optimal threshold value was 3.04,the sensi?tivity of NLR in assessing RA activity was 81.40%,and the specificity was 68.20%.Conclusion NLR is a new inflammatory marker which could be used to assess disease activity in patients with RA.
