摘　　要：目的探讨近年来贮存式自体输血(PABD)在我院的临床应用情况,为更好的开展PABD工作提供科学依据。方法回顾性分析2016年1月~2018年11月我院骨科开展PABD择期手术的患者228例术前3 d采血400 ml,在术中或术后回输,观察采血前后和手术前后红细胞计数(RBC)、血红蛋白(Hb)、血细胞比容(Hct)、血小板计数(PLT)变化,以及献血后和输血不良反应发生情况。结果患者采血前后与患者输血前后RBC、Hb、白细胞计数(WBC)、凝血酶原时间测定(PT)、Hct、PLT比较,差异无统计学意义(P>0.05)。采血中,共出现2例轻度不良反应者,无严重不良反应发生;且回输自体血期间,所有患者均无输血不良反应发生。结论PABD无输血不良反应,科学安全有效,节约了血液资源;PABD方面还有较大的发展潜力,医院临床输血管理委员会应加大管理力度,促进PABD更好的健康发展。Objective To explore the clinical application of preoperative autologous blood donation(PABD)in recent years in our hospital,and to provide a scientific basis for better PABD work.Methods Retrospective analysis of our hospital were carried out in clinical datas of 228 orthopedics patients of PABD in selective operation from January 2016 to November 2018,drew blood 400 ml in the three days before operation,transfused the blood during or after surgery.The value of red blood cell count(RBC),hemoglobin(Hb),hematocrit(Hct),platelet coun(PLT)before and after blood collection and operation were observed,and statistical analyzed the untoward reactions after blood donation and blood transfusion.Results There was no difference in the number of RBC,Hb,white blood cell count(WBC),prothrombin time measurement(PT),Hct and PLT before and after blood collection(P>0.05).During the blood collection process,2 patients with mild adverse reactions had no serious adverse reactions;and during the return of autologous blood,all patients had no adverse reactions of transfusion.Conclusion PABD has no adverse reaction of blood transfusion,scientific and safe,and saves blood resources.There is still great potential for development in PABD.The hospital clinical blood transfusion management committee should increase management and promote the healthy development of PABD.
