200例酒精性肝病的中医证候证素分布特点分析

摘　　要：目的探索酒精性肝病的中医证候、证素分布特点.方法运用数据挖掘技术,以因子分析等方法,参照《证素诊断量表》、《中医临床诊疗术语·证候部分》国家标准,对200例酒精性肝病患者的证候、证素分布进行统计分析.结果研究获得酒精性肝病常见的7种证候类型及分布,分别为湿热内蕴证(55例,27.5%)、肝郁脾虚证(47例,23.5%)、痰湿内阻证(32例,16.0%)、肝胃郁热证(25例,12.5%)、肝肾阴虚证(18例,9.0%)、脾肾阳虚证(13例,6.5%)、痰瘀互结证(10例,5.0%),前三者证候类型较多见.常见证候要素14个,其中病性证素8个,包括湿、热、气滞、气虚、痰、阴虚、阳虚、血瘀;病位证素6个,包括肝、脾、胃、大肠、胆、肾.结论以因子分析方法等数据挖掘技术所获得的证候分布符合临床实际,能够客观阐释中医证候、证素分布特点.Objective To explore the TCM syndromes and distribution characteristics of alcoholic liver disease.Methods Using data mining technology,factor analysis and other methods,200 cases of alcoholic liver disease were analyzed statistically according to the national standards of Syndrome Diagnostic Scale and Clinical Terminology and Syndrome Pan of TCM.Results Seven common syndromes of alcoholic liver disease were found,including dampness-heat accumulation syndrome(55 cases,27.5%),liver stagnation and spleen deficiency syndrome(47 cases,23.5%),phlegm-dampness internal obstruction syndrome(32 cases,16.0%),liver-stomach depression-heat syndrome(25 cases,12.5%),liver-kidney yin deficiency syndrome(18 cases,9.0%),spleen-kidney yang deficiency syndrome(13 cases,6.5%),phlegm-stasis syndrome(10 cases).5.0%,the first three syndrome types were more common.There were 14 common syndrome elements,of which disease syndrome element 8 Including dampness,heat,qi stagnation,qi deficiency,phlegm,yin deficiency,yang deficiency,blood stasis 6 symptoms,including liver,spleen,stomach,large intestine,gallbladder,kidney.Conclusion The distribution of syndromes obtained by data mining techniques such as factor analysis is in fine with clinical practice and can objectively explain the characteristics of syndrome and syndrome element distribution in traditional Chinese medicine(TCM).
