眼动视频录制系统和视频眼震图在良性阵发性位置性眩晕诊治中的价值比较

摘　　要：目的比较眼动视频录制系统(EMRS)与视频眼震图(VNG)对良性阵发性位置性眩晕(BPPV)患者诊治的价值.方法回顾性分析2018年1月至12月期间浙江省邵逸夫医院神经内科门诊确诊的147例BPPV患者,按照检查方法不同分为两组,一组予以VNG检查,另一组予以EMRS检查,分析两者在BPPV定位和治疗中的作用.结果147例患者中,82例行EMRS检查,65例行VNG检查.两者对BPPV患者总检出率(91.46%、92.31%)差异无统计学意义(P>0.05),对后半规管、水平半规管检出率差异无统计学意义(P均>0.05),对管石症、嵴顶结石症检出率差异也无统计学意义(P均>0.05).复位20min后复查位置诱发试验,EMRS组的总有效率与VNG组相比差异无统计学意义(P>0.05).结论EMRS与VNG均能有效诊断BPPV,但EMRS操作简单,携带方便,成本低,值得临床推广应用.Objective To compare the value of eye movement recording goggles(EMRS)and video nystagmogram(VNG)in the diagnosis and treatment of patients with benign paroxysmal positional vertigo(BPPV).Methods 147 patients with BPPV diagnosed in the department of neurology outpatient department of Sir Run Run Shaw Hospital during January 2018 to December 2018 were retrospectively analyzed in this study and divided into two groups according to different examination methods.One group was examined by VNG,the other group by EMRS,and their roles in BPPV localization and treatment were analyzed.Results Among 147 patients,82 cases routined EMRS check and 65 cases routined VNG checks.There was no significant difference in the total detection rate(91.46%,92.31%)between the two groups(P>0.05).There was also no significant difference in the detection rate of posterior semicircular canal and horizontal semicircular canal(P>0.05).There was no significant difference in the detection rate of semicircular duct calculi and semicircular crest calculi(P>0.05).Reexamination of the position-induced test 20 minutes after reduction,the total effective rate of EMRS group was not statistically significant compared with that of VNG group(P>0.05).Conclusion Both EMRS and VNG can effectively diagnose BPPV,but EMRS is simple to operate,convenient to carry,low cost and worthy of clinical application.
