通淋止血汤联合托特罗定治疗经尿道前列腺电切术后膀胱过度活动症临床疗效评价

摘　　要：目的:观察通淋止血汤联合托特罗定治疗经尿道前列腺电切术(TURP)后膀胱过度活动症(OAB)的临床疗效。方法:选择2017年10月~2019年6月成都中医药大学附属医院泌尿外科收治的84例经尿道前列腺电切术后膀胱过度活动症患者,随机分成实验组、中药组和西药组,每组各28例。实验组口服中药通淋止血汤联合托特罗定缓释片,中药组单纯服用中药通淋止血汤,西药组单纯服用托特罗定缓释片。观察三组病例治疗前后膀胱过度活动症症状评分(OABSS)、国际前列腺症状评分(IPSS)、生活质量指数(QOL)等指标,同时记录患者治疗效果、药物不良反应及不良事件等。结果:实验组总有效率为85.71%,高于中药组总有效率67.85%和西药组总有效率78.57%(P值均<0.05)。三组病例治疗后白天排尿次数、夜间排尿次数、尿急、急迫性尿失禁及OABSS评分均较治疗前下降(P值均<0.05);且实验组较中药组、西药组下降分值更显著(P值均<0.05)。三组病例治疗后IPSS评分及QOL评分均较治疗前下降(P值均<0.05);且实验组较中药组、西药组下降分值更明显(P值均<0.05)。结论:通淋止血汤联合托特罗定治疗经尿道前列腺电切术后膀胱过度活动症疗效肯定,能有效改善TURP术后排尿刺激症状,提高生活质量,用药安全可靠。Objective:To investigate the effect of Tonglin Zhixue Decoction combined with Tolterodine on overactive bladder after transurethral resection of prostate. Methods:From October 2017 to June 2019, 84 cases of OAB after TURP treated in Department of Urology, the Teaching Hospita1 of Chengdu University of Traditional Chinese Medicine were selected and randomly divided into three groups, an experimental group, a traditional Chinese medicine group and a western medicine group, with 28 cases in each group. The patients in the received Tonglin Zhixue Decoction combined with Tolterodine treatment, and those in the traditional Chinese medicine group were treated with only Tonglin Zhixue Decoction, and meanwhile the western medicine group were treated with only Tolterodine. The clinical effect, OAB Symptom Scores, IP Symptom Scores, the Quality of Life were recorded and compared. Results:The total response rate in the experimental group was significantly higher than that in the other groups(P<0.05). After treatment, the frequency of urinary, nocturia, urinary urgency, the UUI, and the OABSS were declined in three groups at the sametimes(P<0.05), especially in the experimental group(P<0.05). The QOL and IPSS in the experimental group were markedly lower than those in the other groups(P<0.05). Conclusion:Tonglin Zhixue Decoction combined with Tolterodine on overactive bladder after transurethral resection of the prostate was effective. It can improve the clinical symptoms of patients and reduce the incidence of adverse reactions.
