超声引导下水针刀疗法治疗腰椎间盘突出症的临床观察

摘　　要：【目的】探讨超声引导下水针刀疗法治疗腰椎间盘突出症的临床疗效及其安全性。【方法】将60例腰椎间盘突出症(L4/5)患者随机分为试验组和对照组,每组各30例。对照组患者给予常规穿刺水针刀治疗,试验组患者给予B超引导下水针刀治疗,每周治疗1次,2次为1个疗程。观察2组患者治疗前和治疗后1周、1个月、3个月疼痛视觉模拟量表(VAS)评分和Oswestry功能障碍指数(ODI)评分的变化情况及不良反应发生情况。【结果】(1)治疗后各个观察时点,2组患者VAS评分和ODI评分均较治疗前明显降低(P<0.05),且试验组的降低作用均明显优于对照组,差异均有统计学意义(P<0.05)。(2)不良反应情况:对照组有3例患者出现局部小血肿,而试验组未见明显不良反应。【结论】相对于常规穿刺水针刀治疗,超声引导下的水针刀疗法治疗腰椎间盘突出症有助于提高临床疗效,减少不良反应。Objective To observe the clinical efficacy and safety of hydro-acupuncture and acupotomy guided by ultrasound in treating prolapse of lumbar intervertebral disc.Methods A total of 60 patients with prolapse of lumbar intervertebral disc(L4/5)were randomly divided into trial group and control group,30 cases in each group.Patients in the control group received conventional hydro-acupuncture and acupotomy(a minimally-invasive needling therapy for the diseases of orthopedics and traumatology with the action of dissolution and with injection of medicine),and the trial group were treated with hydro-acupuncture and acupotomy guided by ultrasound type B,once a week and 2 times constituting one treatment course.Before treatment and one week,one month and 3 months after treatment,the Visual Analogue Scale(VAS)pain scores and Oswestry Disability Index(ODI)scores in the two groups were observed,and the adverse reaction in the two groups was also monitored during the trial.Results(1)At various time points after treatment,VAS pain scores and ODI scores in the two groups were markedly decreased(P<0.05 compared with those before treatment),and the decrease in the trial group was superior to that in the control group(P<0.05).(2)Three cases of the control group had small local hematoma,but there was no obvious adverse reaction occurring in the trial group.Conclusion In comparison with conventional hydro-acupuncture and acupotomy,ultrasound-guided hydro-acupuncture and acupotomy can enhance the clinical efficacy for prolapse of lumbar intervertebral disc and reduce the adverse reaction.
