内镜下不同套扎方法治疗食管静脉曲张的对比分析

摘　　要：[目的]对比内镜下六环单次套扎、六环间断2次套扎及十二环连续套扎治疗食管静脉曲张的近期疗效、安全性及远期效果。[方法]将92例肝硬化并食管静脉曲张患者,根据当时治疗情况分3组,六环单次套扎组51例,六环间断2次套扎组21例,十二环连续套扎组20例,对比观察3组疗效、并发症、住院时间、住院费用。[结果]①总疗效:3组近期总疗效基本相同(P>0.05),六环间断2次套扎组及十二环连续套扎组远期疗效优于六环单次套扎组,两两相比差异有统计学意义(P<0.05);②近期并发症:3组治疗后近期并发症基本相同(P>0.05),但胸骨后疼痛在十二环连续套扎组更多见(P<0.05);③住院时间及住院费用:十二环连续套扎组与六环单次套扎组住院时间及住院费用比较差异无统计学意义(P>0.05),均明显少于六环间断2次套扎组(P<0.05)。[结论]治疗肝硬化并食管静脉曲张的3种套扎方法近期疗效及并发症相当,十二环连续套扎及六环间断2次套扎远期疗效均优于六环单次套扎,但十二环连续套扎较六环间断2次套扎住院时间短、费用低,值得临床推广应用。[Objective]To research the effect and safety of esophageal varices treatments with six-loop ligation,intermittent two ligation and twelve-loop continuous ligation under endoscopy.[Methods]Ninety-two patients with hepatic cirrhosis with esophageal varices were allocated into three groups,six-loop group(n=51),intermittent two ligation group(n=21),twelve-loop continuous ligation group(n=20).Comparative analysis of the effect,applications,hospital stay time and hospitalization expenses were performed.[Results]①Total effect:short term efficacy was not statistical different among three groups(P>0.05),but intermittent two ligation and twelve-loop continuous ligation’s long term efficacy were better than six-loop ligation.②Short-term applications:there was no statistical difference among three groups(P>0.05),but the incidence of chest pain was higher in twelve-loop continuous ligation(P<0.05).③ Hospital stay time and hospitalization expenses:there was no significant difference between twelve-loop continuous ligation and six-loop ligation(P>0.05),they both had significant difference with intermittent two ligation groups(P<0.05).[Conclusions]The short-term effect among three groups were not different,but intermittent two ligation and twelve-loop continuous ligation’s long term efficacy is better than six-loop ligation.Furthermore,twelve-loop continuous ligation group’s hospital stay time and hospitalization expenses are lower than intermittent two ligation.
