摘  要:目的通过模拟临床盆底电刺激(electrical stimulation,ES)治疗压力性尿失禁(stress urinary incontinence,SUI)的方法,分析ES对小鼠阴道前壁中Calpain 2和胶原的代谢影响。方法实验分为对照组(CON组)、SUI组、SUI+ES 50 Hz组,每组20只小鼠。构建小鼠SUI模型并进行造模评价,ES后取阴道前壁组织,应用Western Blot和RT-PCR方法比较3组组织中Calpain 2和胶原的表达变化,并在蛋白水平分析各组中Calpain 2与胶原Ⅰ、胶原Ⅲ相关性。结果 SUI+ES 50 Hz组小鼠阴道前壁Calpain 2的蛋白表达含量及mRNA均较SUI组显著升高(P<0.05),SUI组胶原Ⅰ、Ⅲ的蛋白表达、mRNA水平与CON组相比较显著降低(P<0.05),SUI+ES 50 Hz组经ES治疗后胶原Ⅰ、Ⅲ蛋白、mRNA表达水平有所升高(P<0.05)。且在蛋白水平SUI+ES 50 Hz组中Calpain 2与胶原Ⅰ、胶原Ⅲ呈明显正相关(r=0.7251,P=0.0003;r=0.6869,P=0.0008)。结论 ES治疗可激活SUI盆底组织中Calpain 2蛋白,增加胶原含量;且在ES治疗SUI中,Calpain 2的变化与胶原变化密切相关,可能参与其治疗机制。Objective The effects of electrical stimulation(ES) on the metabolism of Calpain 2 and collagen in the anterior wall of the vagina were analyzed by simulating clinical pelvic floor electrical stimulation in the treatment of stress urinary incontinence(SUI). Methods The experiment was divided into control group(CON group), SUI group and SUI+ES 50 Hz group, with 20 mice in each group. The mouse SUI model was constructed and evaluated. The anterior vaginal wall tissue was taken after ES. The expression changes of Calpain 2 and collagen in the three groups were compared by Western Blot and RT-PCR. The correlation between Calpain 2 and collagen Ⅰ and collagen Ⅲ in each group was analyzed at the protein level. Results The protein expression and mRNA of Calpain 2 in the vaginal anterior wall of SUI+ES 50 Hz group were significantly higher than those in SUI group(P<0.05). The protein expression and mRNA levels of collagen Ⅰ and Ⅲ in SUI group were significantly lower than those in CON group(P<0.05), the expression levels of collagen Ⅰ, Ⅲ protein and mRNA were increased in SUI+ES 50 Hz group after electrical stimulation(P<0.05). There was a significant positive correlation between Calpain 2 and collagen Ⅰ and collagen Ⅲ in the SUI+ES 50 Hz group(r=0.7251, P=0.0003, r=0.6869, P=0.0008). Conclusion ES can activate Calpain 2 protein in the pelvic floor tissue of SUI and increase collagen content.
