苏州市无偿献血者与O型反定型细胞反应的抗体筛查分析

摘　　要：目的:筛查分析苏州市中心血站献血者与O型反定型细胞反应的抗体。方法:对2015-01-01—2018-11-30苏州市中心血站采集的342371例无偿献血者血样进行O细胞试验初筛不规则抗体及鉴定,利用χ^2检验和Fisher精确概率法统计分析实验结果。结果:342371例无偿献血者血样中,不规则抗体阳性者288例,阳性率为0.084%。同一性别、不同年龄段不规则抗体阳性率差异无统计学意义(男性P=0.284,女性P=0.271);18~25岁、26~35岁及36~45岁的献血者中,女性不规则抗体阳性率高于男性,差异有统计学意义(3组均P=0.000)。同一性别、不同血型不规则抗体阳性率差异有统计学意义(男性P=0.000,女性P=0.001)。结论:采供血机构有必要对不规则抗体进行筛查,提高血型鉴定和交叉配血的效率,尽量减少抗筛阳性的血液进入临床,进一步提高输血安全性。Objective:To screen and analyze antibodies reactive with O-type cells from voluntary blood donors in Suzhou Blood Center.Method:"O cell test"and antibodies identification were performed on 342371 blood samples collected from Suzhou Blood Center from January 1,2015 to November 30,2018.The chi-square test and Fisher’s exact probability method were used for statistical analysis.Result:Data from 342371 voluntary blood donors were analyzed,from which 288 irregular antibodies were detected(0.084%).There was no significance in the positive rate of irregular antibodies in different age groups between the same sex(male P=0.284,female P=0.271);18 to 25 years old,26 to 35 years old and 36 to 45 years old among the blood donors,the antibody positive rate of females was higher than that of males,and the difference was statistically significant(P=0.000 for all three groups).There was a statistically significant difference in the positive rate of irregular blood antibodies in different blood types(P=0.000 for males and P=0.001 for females).Conclusion:To improve the efficiency of blood type identification and cross-matching and minimize the blood of screening-positive blood,in addition to further improve the safety of blood transfusion,it is necessary for the blood collection and supply institutions to screen irregular antibodies.
