后入路椎板切除减压术联合椎弓根钉内固定术治疗胸腰椎段脊椎骨折伴脊髓损伤的效果

摘　　要：目的 :探讨使用后入路椎板切除减压术联合椎弓根钉内固定术治疗胸腰椎段脊椎骨折伴脊髓损伤的临床效果。方法 :选取2017年12月至2018年12月在宁夏医科大学附属总医院进行治疗的67例胸腰椎段脊椎骨折伴脊髓损伤患者作为研究对象,并将其随机分为REF组(n=34)和OBS组(n=33)。对REF组患者使用前入路椎板切除减压术联合椎弓根钉内固定术进行治疗。对OBS组患者使用后入路椎板切除减压术联合椎弓根钉内固定术进行治疗。然后,比较接受治疗后3个月两组患者生活质量的评分及其治疗的效果。结果 :接受治疗后3个月,OBS组患者生活质量的评分及其治疗的总有效率均高于REF组患者,P<0.05。结论 :对胸腰椎段脊椎骨折伴脊髓损伤患者使用后入路椎板切除减压术联合椎弓根钉内固定术进行治疗的效果较好,可有效地促进其术后的康复。objective:to investigate the clinical effect of posterior approach laminectomy and decompression combined with pedicle screw fixation in the treatment of thoracolumbar vertebral fracture with spinal cord injury.Methods:from December 2017 to December 2018,67 patients with thoracolumbar vertebral fractures and spinal cord injuries treated in the affiliated general hospital of ningxia medical university were selected as the study subjects and randomly divided into REF group(n=34)and OBS group(n=33).Patients in the REF group were treated with anterior approach laminectomy and decompression combined with pedicle screw fixation.Patients in the OBS group were treated with posterior approach laminectomy and decompression combined with pedicle screw fixation.Then,quality of life scores and treatment effects were compared between the two groups at 3 months after treatment.Results:3 months after treatment,the quality of life score and the total effective rate of treatment in OBS group were higher than those in REF group(P<0.05).Conclusion:posterior approach laminectomy and decompression combined with pedicle screw fixation is effective in the treatment of thoracolumbar vertebral fracture with spinal cord injury.
