益气行水汤改善肛肠术后尿潴留临床观察

摘　　要：目的观察益气行水汤改善肛肠术后尿潴留的效果及其应用价值。方法选择2017年1月—2018年7月接受治疗的90例肛肠术后尿潴留患者作为研究对象,按照随机数字表法分为试验组和对照组,各45例。2组均采取流水声诱导、腹部热敷、心理暗示等非药物治疗手段,对照组在此基础上予患者盐酸坦索罗辛缓释胶囊联合盐酸特拉唑嗪片治疗,试验组在对照组的基础上予以患者自拟益气行水汤口服(包括黄芪、茯苓、桑白皮等14种中药)。比较2组患者术后开始治疗前和治疗24 h后肛门疼痛情况(采用视觉模拟评分法)、术后2组患者的排尿情况(首次排尿时间、24 h排尿次数和24 h平均尿量)、临床疗效和治疗有效率。结果治疗24 h后,2组患者疼痛评分均下降且试验组疼痛评分显著低于对照组[(1.54±0.49)分VS(3.61±0.44)分];试验组首次排尿时间、24 h排尿次数短/少于对照组,24 h平均尿量多于对照组;试验组患者临床疗效优于对照组,且治疗有效率高于对照组(治疗有效率:97.78%VS71.11%),以上差异均有统计学意义(P<0.05)。结论益气行水汤有利于改善患者肛肠术后尿潴留的临床症状,显著提高临床疗效及治疗有效率,减轻疼痛情况。Objective To observe the effect and application value of Yiqi Xingshui decoction in improving urinary retention after anorectal surgery.Methods Ninety patients with urinary retention after anorectal surgery who were treated in our hospital from January 2017 to July 2018 were selected as the study subjects.They were randomly divided into experimental group(n=45)and control group(n=45).The control group was treated with tamsulosin hydrochloride sustained release capsule combined with terazosin hydrochloride tablets.The experimental group was treated with Yiqi Xingshui decoction(including 14 kinds of traditional Chinese medicines such as Astragalus membranaceus,Poria cocos and mulberry bark)on the basis of the control group.The anal pain before and after 24 hours of treatment(visual analog scoring method),urination(first urination time,24 hours of urination frequency and 24 hours average urine volume),clinical efficacy and treatment efficiency were compared between the two groups.Results After 24 hours of treatment,the pain scores of both groups decreased and the pain scores of the experimental group were significantly lower than those of the control group[(1.54±0.49)and VS(3.61±0.44)].The first urination time,24 hours urination frequency of the experimental group were shorter/less than those of the control group,and the 24 hours average urine volume of the experimental group was more than that of the control group.The clinical effect of the experimental group was better than that of the control group,and the effective rate of the experimental group was higher than that of the control group(97.78%VS 71.11%),and the difference was statistically significant(P<0.05).Conclusion Yiqi Xingshui decoction can improve the clinical symptoms of urinary retention after anorectal surgery,significantly improve the clinical effect and effective rate,and relieve pain.
